Predictive value of prenatal plasma fibrinogen and thrombus elastogram parameters for postpartum hemorrhage in elderly women with cesarean section
Objective To study the predictive value of prenatal plasma fibrinogen(Fib)and thrombus elastogram parameters for postpartum hemorrhage in elderly women with cesarean section.Methods A total of 104 elderly pregnant women with cesarean section in the Central Hospital of Xinxiang were selected for this study.Before delivery,plasma Fib and thromboxogram parameters were used to predict postpartum hemorrhage.The actual postpartum situation was taken as the standard to evaluate the parameters of plasma Fib and thromboxogram and their combined prediction results,and coagulation indexes and thromboxogram parameters were compared between the postpartum hemorrhage group and the non-postpartum hemorrhage group.Results Among the 104 elderly pregnant women with cesarean section,postpartum hemorrhage(positive)occurred in 54 cases,non-postpartum hemorrhage(negative)in 50 cases,plasma Fib prenatal prediction was positive in 40 cases and negative in 64 cases,thrombologram parameters prenatal prediction was positive in 44 cases and negative in 60 cases,and the combined prenatal prediction was positive in 51 cases and negative in 53 cases.The accuracy and sensitivity of the combined prenatal prediction were higher than those of plasma Fib and thromboelastogram parameters(P<0.05).The Fib of postpartum hemorrhage group was significantly lower than that of non-postpartum hemorrhage group(P<0.05),but there was no significant difference in other indexes(P>0.05).Compared with the non-postpartum hemorrhage group,the K value of the postpartum hemorrhage group was higher and the MA value was lower,the differences were significant(P<0.05),and the other parameters were not significantly different between the two groups(P>0.05).Conclusions Prenatal plasma Fib combined with thromboelastogram parameters has good prediction value for postpartum hemorrhage in elderly women with cesarean section,and its prediction efficiency is high(high accuracy and sensitivity),and prenatal plasma Fib level and the abnormal K and MA values in thromboelastogram parameters can also be combined to assist judgment,so as to provide a basis for clinical prediction of postpartum hemorrhage.
