Effect of sclerosing agent injection combined with endoscopic variceal ligation on liver cirrhosis complicated by esophageal and gastric fundus variceal bleeding
Objective To investigate the efficacy of sclerotherapy injection combined with endoscopic variceal ligation on liver cirrhosis complicated by esophageal and gastric fundus variceal bleeding(EGVB).Methods Eighty patients with liver cirrhosis complicated with EGVB admitted to Jiaozuo People's Hospital from October 2020 to October 2023 were selected as the research subjects.They were divided into a control group and a study group according to the random number table method,with 40 cases in each group.The control group received endoscopic ligation treatment,and the study group used sclerotherapy injection combined with endoscopic ligation for treatment.The treatment efficacy,hemostasis time,transfusion volume,length of hospital stay,variceal recurrence rate,rebleeding rate,mortality rate,portal vein pressure(PPV),portal vein diameter(DPV),portal vein flow velocity(VPV),and incidence of complications between the two groups were compared.Results The total effective rate of treatment in the study group(95.00%,38/40)was higher than that in the control group(80.00%,32/40),and the difference was significant(P<0.05).The study group had less hemostatic time and transfusion volume than the control group,and the difference was significant(P<0.05).There was no significant difference in the length of hospital stay between the two groups(P>0.05).During the 6-month follow-up period,the recurrence rate of varicose veins in the study group(10.00%,4/40)was lower than that in the control group(27.50%,11/40),and the difference was significant(P<0.05).There was no significant difference in the rebleeding rate and mortality rate between the two groups(P>0.05).After treatment,the serum GAS and MTL levels in both groups decreased,and those of the study group was significantly lower than those of the control group,with significant difference(P<0.05).There was no significant difference in the total incidence of complications between the control group(15.00%,6/40)and the study group(17.50%,7/40)(P>0.05).Conclusions Sclerotherapy injection combined with endoscopic variceal ligation shows better therapeutic effects than endoscopic variceal ligation in the treatment of liver cirrhosis complicated by EGVB,including higher treatment efficiency,shorter hemostasis time,less blood transfusion,lower variceal recurrence rate,and better improvement of gastrointestinal hormone levels.
