外侧复合体增强缝合在踝关节距腓前韧带损伤修复中的应用效果

目的 观察外侧复合体增强缝合在踝关节距腓前韧带损伤修复中的应用效果.方法 选择2019年1月至2022年12月郑州人民医院收治的82例踝关节距腓前韧带损伤患者为研究对象,采用随机数字表法分为对照组与研究组,每组41例.对照组给予普通缝合修复,研究组给予外侧复合体增强缝合修复.比较两组患者围术期指标(术中出血量、手术时间)、踝关节功能、活动度、美国足踝外科协会(AOFAS)总分、疼痛情况[视觉模拟评分法(VAS)评分]及关节稳定性(距骨倾斜角、距骨前移距离).结果 两组术中出血量与手术时间比较,差异未见统计学意义(P＞0.05).术后,两组踝关节功能、活动度及AOFAS总分均高于术前,且研究组高于对照组,差异有统计学意义(P＜0.05).术后,两组VAS评分均低于术前,但组间比较差异未见统计学意义(P＞0.05).术后,两组距骨倾斜角与距骨前移距离均小于术前,且研究组小于对照组,差异有统计学意义(P＜0.05).结论 外侧复合体增强缝合应用于踝关节距腓前韧带损伤修复中能够提高踝关节稳定性,改善踝关节功能,缓解关节疼痛症状.
Abstract
Objective To observe the application effect of lateral complex enhanced suture in the repair of anterior talofibular ligament injury of ankle joint.Methods A total of 82 patients with anterior talofibular ligament injury of ankle joint admitted to Zhengzhou People's Hospital from January 2019 to December 2022 were selected as the study objects,and were divided into control group and study group by random number table method,with 41 cases in each group.The control group received conventional suture repair,and the study group received lateral complex enhanced suture repair.The perioperative parameters(intraoperative blood loss,operative time),ankle function,range of motion,American Association of Foot and Ankle Surgery(AOFAS)total score,pain[visual analogue scale(VAS)score]and joint stability(talus tilt angle,talus advance distance)were compared between the two groups.Results There was no significant difference in the amount of blood loss and operation time between the two groups(P＞0.05).After surgery,the ankle joint function,range of motion and total AOFAS score in both groups were higher than those before surgery,and those of the study group were higher than those of the control group,the differences were significant(P＜0.05).After operation,the VAS scores in both groups were lower than those before operation,but there was no significant difference between the two groups(P＞0.05).After operation,the inclination angle and advance distance of talus of the two groups were lower than those before operation,and those of the study group were smaller than those of the control group,the differences were significant(P＜0.05).Conclusions Application of lateral complex enhanced suture for concomitant anterior talofibular ligament injury following ankle injury can improve stability and function of the ankle joint,alleviate pain.
