Predictive value of HALP score and NLR before treatment for the prognosis of esophageal cancer patients undergoing radiotherapy and chemotherapy
Objective To explore the prognostic value of hemoglobin,albumin,lymphocyte,and platelet(HALP)scores and neutrophil-to-lymphocyte ratio(NLR)in patients with esophageal cancer undergoing radiotherapy and chemotherapy before treatment.Methods One hundred and six patients with esophageal cancer who underwent radiotherapy and chemotherapy in the Department of Oncology at our hospital from January 2019 to December 2021 were selected as the study subjects.According to the efficacy evaluation,they were divided into a good prognosis group(n=55)and a poor prognosis group(n=51).Clinical data of two groups were collected,and relationships of HALP score and NLR with the clinical pathological characteristics of esophageal cancer patients were analyzed.Kaplan-Meier method was used for survival analysis,and Pearson correlation analysis was used to investigate the correlation of HALP score and NLR with postoperative complications of esophageal cancer patients after radiotherapy and chemotherapy.Receiver operating characteristic(ROC)curve analysis was used to evaluate the prognostic value of HALP score,NLR,and squamous cell carcinoma antigen(SCC)in esophageal cancer patients.Results Compared with the poor prognosis group,the HALP score of esophageal cancer patients in the good prognosis group increased,while the NLR decreased(P<0.05).The HALP score and NLR were closely related to adverse prognostic pathological features such as TNM staging,tissue differentiation degree,and lymph node metastasis in esophageal cancer patients(P<0.05).The HALP score was negatively correlated with malnutrition,radiation-induced esophagitis,and pulmonary infection in patients with esophageal cancer(r=-0.614,-0.437,-0.541,P<0.001).The NLR was positively correlated with pulmonary infection and radiation-induced esophagitis in patients(r=0.469,0.410,P<0.001).Combined detection of HALP score,NLR,and SCC predicted the area under the prognosis curve of esophageal cancer to be 0.883(95%CI:0.806-0.937),with a maximum Jordan index of 0.8120,sensitivity of 0.8889 and specificity of 0.9231.Conclusion The HALP and NLR scoring systems are valuable biomarkers for prognostic evaluation of esophageal cancer,and can provide reference for clinical healthcare professionals to develop adverse prognostic strategies for esophageal cancer.
