微创弹性髓内钉治疗小儿股骨干骨折疗效分析

摘要
目的 探讨微创弹性髓内钉(ESIN)治疗小儿股骨干骨折的疗效.方法 选取解放军184医院自2014年1月至2016年7月收治的30例小儿股骨干骨折患儿,采用ESIN固定治疗,观察手术切口、手术时间以及术中出血量.对患儿进行随访,探讨其疗效、膝关节功能恢复以及并发症发生情况.结果 30例患者均获得满意效果,手术切口约1.5 ～2.0 cm;手术时间30 ～ 50 min;术中出血20～30 ml.随访12 ～ 30个月,平均随访16个月.对术后疗效进行评价,优28例,良2例,优良率100.0％.2例为膝关节活动受限20°～45°,在下肢关节被动活动器(CPM)辅助下功能练习6个月后,膝关节功能恢复正常,无弹性髓内钉断裂、骨不连现象发生.结论 ESIN在治疗小儿股骨干骨折中具有固定可靠、创伤小、操作简便、治疗效果佳等优点.
Abstract
Objective To investigate the curative effect of minimally invasive elastic stable intramedullary nail(ESIN) treatment of femoral fractures in children.Methods A retrospective study was performed on 30 cases of pediatric femoral fractures from January 2014 to July 2016.Using ESIN fixation treatment,the surgical incision,operation time,and intraoperative bleeding were observed.The children were followed up,the curative effect,recovery function of knee and complications were investigated.Results All 30 cases obtained satisfactory results,the surgical incision was about 1.5 to 2.0 cm;operation time was 30 to 50 minutes;the intraoperative bleeding was about 20 to 30 ml.Children were followed up for 12 to 30 months,an average of 16 months.The postoperative curative effect was evaluated,superior in 28 cases,good in 2 cases,and the excellent and good rate was 100％.Two cases of knee joint activities limited 20 ° to 45 °,assisted by CPM device in lower extremity joints passive activities under the auxiliary function exercises after six months,the knee joint function recovered to normal,no phenomenon of elastic intramedullary nail fracture or bone nonunion occurred.Conclusion ESIN treatment of pediatric femoral fractures has the advantages of reliable fixation,small trauma,simple operation and good treatment effect,which is worthy of clinical promotion.
