Value of two methods of 18 F-fluorodeoxyglucose PET/CT in the evaluation of interim therapeutic efficacy and prognosis of diffuse large B-cell lymphoma
Objective To investigate the efficacy of 18F-fluorodeoxyglucose(18F-FDG)PET/CT Deauville 15-point scale and ΔSUVmax%method in the interim evaluation of chemotherapy in patients with diffuse large B-cell lymphoma(DLBCL)and the value of predicting the curative effect in patients after 12 monthes.Methods The clinical and imaging data of 42 DLBCL patients admitted to General Hospital of Northern Theater Command from April 2019 to November 2022 were retrospectively analyzed.Receiver operating characteristic(ROC)curve analysis was used to classify the optimal cut-off value of ΔSUVmax%for determining the curative effect.Lugano evaluation standard as the"gold standard",the consistency of Deauville 5-point scale method,ΔSUVmax method and Lugano criteria for evaluation of patients'interim chemotherapy treatment was compared.The sensitivity and specificity of ΔSUVmax method and Deauville 5-point scale method were compared.The Deauville 5-point scale method and ΔSUVmax method were analyzed to predict disease remission after 12 months.Results The Kappa value of the consistency test between Deauville 5-point scale and Lugano eval-uation criteria was 0.538(P =0.011).The Kappa value of the consistency test between ΔSUVmax method and Lugano evaluation cri-teria was 0.769(P<0.001).The sensitivity and specificity of ΔSUVmax method were 83.33%and 93.33%,respectively higher than those of Deauville5-point scale(50.00%and 82.35%),but the difference was not statistically significant(P>0.05).Deauville 5-point scale method:the complete remission rate of negative patients was70.59%(12/34)after12 months,which was higher than that of positive patients(25.00%,2/8),and the difference was statistically significant(P<0.05).ΔSUVmax method:the complete remis-sion rate of negative patients after 12 months was 80.00%(24/30),higher than that of positive patients(16.67%,2/12),the differ-ence was statistically significant(P<0.05).Conclusion Mid-term PET/CT based visual analysis Deauville5-point scale and semi-quantitative analysis ΔSUVmax method have higher value in evaluating the efficacy of mid-term chemotherapy in DLBCL patients and predicting the efficacy after 12 months of chemotherapy,and ΔSUVmax method has higher consistency with Lugano standard.
