Diagnostic value of 7 tumor-associated autoantibodies combined with 4 tumor markers for benign and malignant pulmonary nodules
Objective To investigate the clinical diagnostic value of 7-tumorassociated autoantibodies(7-AABs)combined with 4 tumor markers in benign and malignant pulmonary nodules.Methods The clinical data of 370 patients who underwent surgical re-section of pulmonary nodules in General Hospital of Northern Theater Command from January 2018 to January 2023 were collected.The patients were divided into malignant nodule group(n=287)and benign nodule group(n=83).The clinical data of malignant nodule group and benign nodule group were compared and analyzed.Enzyme-linked immunosorbent assay was used to detect the expression levels of 7-AABs(p53,PGP9.5,SOX2,GAGE7,GBU4-5,MAGE,CAGE)and four tumor markers[carcinoembryonic antigen(CEA),cytokeratin 19 fragment(CYFRA 21-1),squamous cell carcinoma antigen(SCCA),neuron-specific enolase(NSE)]in pe-ripheral blood of the study subjects,and the receiver operating characteristic curve(ROC)was plotted and the area under the curve(AUC)was calculated.To analyze the diagnostic efficacy of 7-AABs or four tumor markers in malignant pulmonary nodules and the combined diagnostic efficacy of both.Results There were significant differences in smoking history,nodule size and nodule nature be-tween the malignant nodule group and the benign nodule group(P<0.05).In the malignant nodules group,the proportion of nodules with the size of 9 to 19 millimeters and 20 to 30 millimeters was compared with that of nodules with the size ≤8 millimeters,and the difference was statistically significant(P<0.05).The proportion of ground glass nodules and mixed nodules was statistically signifi-cant compared with solid nodules(P<0.05).In 7-AABs,the expression levels of p53,SOX2,GAGE7 and GBU4-5 in malignant nod-ules group were higher than those in benign nodules group,with statistical significance(P<0.05).Among the 4 tumor markers,the SCCA expression level in the malignant nodules group was higher than that in the benign nodules group,and the difference was statisti-cally significant(P<0.05).The diagnostic efficacy of the combined diagnosis of four tumor markers for malignant nodules(AUC=0.596,P<0.05)was slightly worse than that of the combined diagnosis of 7-AABs(AUC=0.706,P<0.05),but the combined di-agnosis of the two tumor markers significantly improved the diagnostic efficacy of malignant nodules(AUC=0.737,P<0.05).Conclusion 7-AABs combined with four tumor markers has more clinical significance for the identification of benign and malignant pulmonary nodules and the early screening and diagnosis of lung adenocarcinoma than any single marker.It can be used as an auxiliary examination means for the identification of lung nodules,and it is non-invasive and easy to perform,and can be an effective measure for the early screening of lung cancer.
