Development and Application of a Prior Information Collection Tool for Chinese Older Adults:A Field Study Based on Institution A
Promoting integrated care for older people(ICOPE)requires resource and information integration of individuals,families,communities and institutions,as well as the establishment of interconnected mechanisms to drive innovation of care models.Therefore,guided by a person-centered care mechanism,the study developed a daily life preference scale suitable for Chinese older adults,with the objective of exploring how to effectively integrate care information resources from multiple players and accurately capture older adults'care preferences.The results of the scale were then compiled to create Preferences for Activity and Leisure(PAL)cards,to ensure the coherence and consistency of care information among different care providers.PAL cards serve as an effective communication tool that provides prior information,helping caregivers reduce communication costs and improve quality of care services.Additionally,this tool can effectively address caregiver turnover,allowing new caregivers to quickly grasp the essentials of caregiving.It is recommended that future studies continue to test the feasibility,acceptability,and applicability of PAL cards,and actively explore online data collection modes to build an online sharing platform while prioritizing information security and privacy protection.
