环钻切除术联合CO2点阵激光治疗凹陷型痤疮瘢痕的临床效果

摘要
目的 评价环钻切除术与CO2点阵激光联合改善面部凹陷型痤疮瘢痕的临床效果及安全性.方法 纳入就诊于2019年2月—2022年2月郑州人民医院医学美容中心的面部凹陷型痤疮瘢痕68例,其中观察组30例,对照组38例.观察组给予环钻切除术联合CO2点阵激光治疗,对照组仅给予CO2点阵激光治疗.治疗结束后6个月,比较两组痤疮瘢痕临床评估量表(ECCA)评分,并进行疗效评估.结果 治疗前观察组患者的瘢痕ECCA评分[50.00(43.75,60.00)]和对照组[47.50(37.50,60.00)]差异无统计学意义(P=0.535).治疗结束后6个月,两组患者ECCA评分均较治疗前显著降低(均P＜0.01),但观察组患者ECCA评分明显低于对照组[20.00(15.00,30.00)比25.00(20.00,30.00),P=0.032],有效率明显高于对照组(93.33％比71.05％,P=0.020).两组患者不良反应发生率无统计学差异(P=1.000).结论 环钻切除术与CO2点阵激光联合较单纯应用CO2点阵激光对于改善面部凹陷型痤疮瘢痕临床效果好,不会增加相关不良反应.
Abstract
Objective To evaluate the efficacy and safety of trephine resection combined with fractional ablative CO2 laser for pitted acne scars.Methods Sixty eight patients with facial pitted acne scars were enrolled from the Medical Cosmetology Center of Zhengzhou People's Hospital from February 2019 to February 2022.The patients were randomly divided into the observation(30 patients)and the control groups(38 patients).The observation group was treated with tre-phine resection combined with CO2 laser,and the control group was treated with fractional ablative CO2 laser alone.The efficacy and the ECCA scale(Echelle d'Evaluation Clinique des Cicatrices d acne)were compared between the two groups after 6-month treatment.Results Before the treat-ment,ECCA scores were comparable between the observation and the control groups[50.00(43.75,60.00)vs.47.50(37.50,60.00),P=0.535].After 6-month treatment,the ECCA scores in both groups were significantly lower than the baseline(both P＜0.01),and the ECCA scores were significantly lower in the observation group[20.00(15.00,30.00)]than in the con-trols[25.00(20.00,30.00)](P=0.032).The effective rate in the observation group was sig-nificantly higher than that in the controls(93.33％vs.71.05％,P=0.020).But adverse reac-tions did not differ significantly between the two groups(P=1.000).Conclusions Trephine re-section combined with fractional ablative CO2 laser is more effective than fractional ablative CO2 la-ser alone for facial pitted acne scars and is safe.
