面颈部播散性皮肤鳞状细胞原位癌1例

报告1例手术联合光动力技术治疗面颈部播散性皮肤鳞状细胞原位癌.患者男,83岁,因面颈部多发色素性增生物7月余就诊.皮肤科检查:面颈部可见散在、边界清楚的数毫米暗红色斑丘疹,部分融合成形状不规则的深褐色斑块,表面可见灰白色鳞屑厚痂.皮损组织病理示:细胞排列紊乱,极性丧失,重度异型增生及挖空细胞,细胞可见病理性核分裂象.诊断为皮肤鳞状细胞原位癌.予手术切除明显隆起、增厚的主体肿瘤病灶,术后再行CO2激光联合光动力治疗散在的多处小病灶.随访1年余,患者无新发或可疑皮损.
Abstract
We report a case of disseminated skin squamous cell carcinoma in situ on the face and neck treated with surgery in combination with photodynamic therapy.An 83-year-old man presented with a 7-month history of multiple pigmented hyperplasia on the face and neck.Derma-tological examination showed that demarcated several millimeter-size dark red papules on the face and neck,part of which merged into irregularly shaped dark brown plaques with thick grayish-white scaly scabs on the surface.Pathology showed the disorganized koilocytes with loss of polarity,dysplasia and pathological mitosis.The diagnosis was squamous cell carcinoma in situ.Following excision of the elevated part of the main tumors,photodynamic therapy with CO2 laser was given.No recurrence or new lesions were observed during 1-year follow up.
