摘要
目的 探讨心脏瓣膜术后患者抗凝治疗依从性现状及其影响因素.方法 采用便利抽样法,选取2023年1月—2024年3月在郑州市第七人民医院心外科行心脏瓣膜手术的患者作为研究对象.采用一般资料调查表、Morisky服药依从性量表(MMAS-8)、抗凝知识问卷、领悟社会支持量表和一般自我效能感量表进行问卷调查,采用多元线性回归探讨心脏瓣膜术后患者抗凝治疗依从性的影响因素.结果 265例心脏瓣膜置换术后患者中,抗凝治疗依从性得分为(6.08±0.92)分,其中53.6%的患者抗凝治疗依从性差.多元线性回归分析结果显示,文化程度、居住情况、医疗费用支付方式、人均月收入和一般自我效能是抗凝治疗依从性的影响因素(P均<0.001),共解释总变异水平的76.7%.结论 心脏瓣膜术后患者的抗凝治疗依从性处于中等偏下水平,文化程度、居住情况、医疗费用支付方式、人均月收入和一般自我效能是影响抗凝治疗依从性的因素.
Abstract
Objective To investigate the current status of adherence to anticoagulation therapy in postoperative heart valve patients and its influencing factors.Methods Patients who underwent heart valve surgery at the Department of Cardiothoracic Surgery,The 7th People's Hospital of Zhengzhou,from January 2023 to March 2024 were selected as the study subjects using the convenience sampling method.Questionnaires were administered using the general information questionnaire,morisky medication adherence scale(MMAS-8),warfarin anticoagulation knowledge scale,comprehension social support scale and general self-efficacy scale,and multivariate linear regression was used to explore the influencing factors of patients'adherence to anticoagulant therapy after heart valve surgery.Results Among 265 postoperative heart valve replacement patients,the mean anticoagulation therapy adherence score was 6.08±0.92,of which 53.6%had poor anticoagulation therapy adherence.The results of multiple linear regression analysis showed that literacy,residence,med-ical payment method,per capita monthly income,and general self-efficacy were the influencing factors of anticoagu-lation therapy adherence(all P<0.001),which explained 77.1%of the total level of variance.Conclusion Ad-herence to anticoagulation therapy in postoperative heart valve patients is at a moderately low level,with literacy,residence,mode of payment for medical care,per capita monthly income,and general self-efficacy are factors for adherence to anticoagulation therapy.
