基于数据挖掘技术分析老年功能性便秘的针灸取穴规律

摘要
目的 基于数据挖掘技术探析现代临床文献中针灸治疗老年功能性便秘的穴位选用特点.方法 收集2003-2023年万方数据库(WanFang)、中国知网数据库(CNKI)、中国生物医学文献数据库(CBM)、维普数据库(VIP)及Pubmed中针灸治疗老年功能性便秘的临床研究文献,利用Office Excel 2019建立腧穴处方数据库,运用中医传承计算平台3.0对腧穴进行关联规则及聚类分析.结果 整理出121篇文献,涉及121条穴位处方,涉及腧穴69个,总取穴频次782次.使用频次较高的腧穴为天枢、上巨虚、足三里跟关元;腧穴较多归足阳明胃经、任脉及足太阳膀胱经;特定穴以募穴、五输穴及下合穴较多;关联规则分析支持度较高的有天枢-上巨虚、天枢-足三里、足三里-上巨虚,核心组方"天枢-上巨虚-足三里-中脘-气海-关元-大肠俞-支沟-三阴交-脾俞-太冲";聚类分析得到3个有效聚类群.结论 针灸治疗老年功能性便秘重视不同经络腧穴配伍应用,处方以特定穴为主,注重俞募配穴、合募配穴,局部取穴与远道取穴相结合,对临床有-定指导意义.
Abstract
Objective To analyze the characteristics of acupoint selection in the treatment of elderly patients with functional constipation in modern clinical literature based on data mining technology.Methods Clinical research literatures on acupuncture treatment of elderly patients with functional constipation were collected from WanFang Database,CNKI Data-base,China Biomedical Literature Database(CBM),VIP database and Pubmed database from 2003 to 2023.Office Excel 2019 was used to establish an acupoint prescription database.Traditional Chinese Medicine Inheritance Computer System 3.0 was used to conduct association rules and cluster analysis on acupoints.Results 121 literatures were sorted out,involving 121 acupoint prescriptions and 69 acupoints,with a total frequency of 782 acupoints.The acupoints with high frequency of use were Tianshu,Shangjuxu,Zusanli and Guanyuan.Most acupoints belonged to the Stomach Meridian of Foot-Yangming,Ren Vessel and Bladder Meridian of Foot-Taiyang.There were more specific acupoints,such as front Mu acupoint,Wushu acupoint and Xiahe acupoint.The association rule analysis showed that Tianshu-Shangjuxu,Tianshu-Zusanli,Zusanli-Shangjuxu had high support degree.The core formula was"Tianshu-Shangjuxu-Zusanli-Zhongwan-Qihai-Guanyuan-Dachan-gshu-Zhigou-Sanyinjiao-Pishu-Taichong".Three effective clusters were obtained by cluster analysis.Conclusion Acupunc-ture treatment of functional constipation in the elderly pay attention to the application of different meridians and acupoints.The prescription mainly focuses on specific acupoints.It emphasizes the matching of Yu acupoint and Mu acupoint,multiple acupoint matching and combination of local acupoint with remote acupoint.It has certain guiding significance for clinical practice.
