Correlation analysis of carotid artery lesions and blood pressure variability in elderly hypertensive patients in a community
Objective To analyze the correlation between carotid artery lesions and blood pressure variability in elderly hypertensive patients over 65 years old in the community,and develop early intervention strategies for the community.Methods From June 2022 to June 2023,460 outpatient elderly patients over 65 years old in a community in Pudong New Area,Shanghai were selected.According to whether they had hypertension,they were divided into hypertension group(HTN group,n=274)and non-hypertension group(NHTN group,n=186).The basic clinical data,laboratory indexes and carotid artery le-sions of the two groups were compared.The correlation between dynamic blood pressure indexes and carotid artery lesions was an-alyzed by Pearson correlation analysis.The influencing factors of carotid artery lesions in elderly hypertensive patients in the com-munity were analyzed by multivariate logistic regression analysis.Results The detection rate of carotid intima-media thickness(IMT)plaques in the HTN group was significantly higher than that in the NHTN group(x2plaque=28.174,P<0.05).The dynam-ic blood pressure indexes,including 24 hSBP,dSBP,nSBP,24 hSBPCV,dSBPCV,and nSBPCV,in the IMT plaque and IMT thickened group were significantly higher than those in the IMT normal group(P<0.05).24 hSBP,dSBP,nSBP,24 hSBPCV,nSBPCV were independent risk factors for carotid IMT lesions.Conclusion There is a certain association between blood pressure variability and carotid artery lesions in elderly hypertensive patients.Systolic blood pressure variability is a risk factor for carotid artery lesions.Early intervention of these risk factors can effectively prevent the occurrence and development of atherosclerosis.
