摘要
目的 探究加味柴胡疏肝汤联合奥卡西平片治疗卒中后癫痫患者的疗效及血清S100B、MBP水平分析.方法 采用随机分组法将我院2019年10月到2021年10月重症医学及神经重症科收治的76例患者分为观察组与对照组,两组各例38例.对照组采用奥卡西平片治疗,观察组在此基础上联合使用加味柴胡疏肝汤治疗.观察两组患者临床疗效与不良反应发生率,比较两组患者治疗前后血清中S100B、MBP、NSE的水平变化以及NIH-SS评分.结果 两组患者治疗后观察组总有效率为94.74％显著高于对照组78.96％(x2=4.145,P=0.041).两组患者治疗前S100B、NSE、MBP水平无统计学差异(P＞0.05).治疗后观察组S100B、NSE、MBP水平较对照组显著降低(P＜0.05).治疗前两组患者NIHSS评分无统计学差异(P＞0.05).治疗后观察组NIHSS评分较对照组显著降低(P＜0.05).观察组不良反应发生率为7.89％略低于对照组13.15％,当不良反应发生率无统计学差异(x2=0.558,P=0.451).结论 加味柴胡疏肝汤联合奥卡西平片能有效提高卒中后癫痫患者治疗、改善S100B、MBP水平、降低不良反应发生率,修复患者神经损伤,提高生活质量.
Abstract
Objective To investigate the effects of modified Chaihu Shugan decoction combined with oxcarbazepine tab-lets on serum S100 calcium-binding protein B(S100B)and myelin basic protein(MBP)levels in patients with post-stroke epilepsy.Methods Seventy-six patients in Critical Care Medicine and Neurocritical Care of our hospital from October 2019 to October 2021 were randomly divided into two groups,each with 38 cases.Control group received oxcarbazepine tablets,based on this,observation group received modified Chaihu Shugan decoction.Then the clinical efficacy,adverse reaction rate,serum levels of S100B and MBP,neuron-specific enolase(NSE),and score of National Institute of Health Stroke Scale(NIHSS)were compared between two groups.Results The overall efficacy rate was 94.74％in observation group,which was obviously higher than 78.96％in control group,with statistical difference(x2=4.145,P=0.041).Baseline S100B,NSE and MBP levels yielded no statistical difference between two groups(P＞0.05),while serum levels of three indicators were decreased in observation group compared with control group(P＜0.05).NIHSS score at baseline demonstrated no sta-tistical difference between two groups,while the post-treatment NIHSS score was lower in observation group than in control group(P＜0.05).The total adverse reaction rate was 7.89％in observation group,which was slightly lower than 13.15％in control group,with no statistical difference(x2=0.558,P=0.451).Conclusion Application of modified Chaihu Shu-gan decoction combined with oxcarbazepine tablets in the treatment of post-stroke epilepsy can effectively ameliorate serum levels of S100B and MBP,reduce adverse reaction rate,promote the recovery of neurological damage,thus improving the quality of life in patients.
