摘要
目的 探究加味柴胡龙牡汤治疗脑卒中的临床疗效分析.方法 采用回顾性分析将本院2019年3月到2021年3月的73例患者分为观察组(n=40)与对照组(n=33),对照组使用常规治疗方法,观察组则在此基础上使用加味柴胡龙牡汤.观察两组的血液流变指标、神经功能缺损评分、临床疗效对比、抑郁状态评分、脑血流灌注指标与血清炎症因子变化.结果 观察组总有效率(92.50%)与对照组总有效率(93.93%)无统计学差异(x2=0.058,P=0.808).2组治疗后,观察组的神经功能损伤状态评分显著低于对照组(P<0.05).治疗前2组患者的生活能力评分无统计学差异(P>0.05).治疗后,观察组的生活能力评分显著高于对照组(P<0.05).两组患者治疗前MTT、TTP、CBV、CBF指标无统计学差异(P>0.05).治疗后观察组MTT、TTP指标较对照组显著降低,且CBV、CBF指标较对照组显著升高(P<0.05).两组患者治疗前IL-6、TNF-α、hsCRP水平无统计学差异(P>0.05).治疗后观察组IL-6、TNF-α、hsCRP水平较对照组显著降低(P<0.05).治疗后,观察组的不良反应发生率(95.00%)略微高于对照组(93.94%),但不具有统计学差异(x2=0.039,P=0.842).
Abstract
Objective To investigate clinical efficacy of modified Chaihu Longmu decoction in treating stroke.Methods Seventy-three patients with stroke in our hospital from March 2019 to March 2021 were retrospectively classified into two groups.Control group(n=33)received routine treatment,based on this,observation group(n=40)received modified Chaihu Longmu decoction.Hemorheological indexes,neurological deficit score,clinical efficacy,depression score,cerebral blood flow perfusion index and serum inflammatory factors were compared between the two groups.Results The total efficacy rate was 92.50%in observation group,which presented no significant difference with 93.93%in control group(x2=0.058,P=0.808).The neurological deficit score demonstrated no significant difference between the two groups at the baseline(P>0.05),and the post-treatment score was significantly lower in observation group than in control group(P<0.05).The score of activity of daily living yielded no significant difference between the two groups at the baseline(P>0.05),and observation group scored notably higher than control group after treatment(P<0.05).Parameters including mean transit time(MTT),time to peak(TTP),cerebral blood volume(CBV)and cerebral blood flow(CBF)showed no statistical difference between two groups before treatment(P>0.05),while observation group had obviously shorter MTT and TTP as well as larger CBV and CBF than those of control group,with statistical difference(all P<0.05).Serum levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and high-sensitivity CRP(hsCRP)presented no statistical difference between two groups before treatment(P>0.05),and the three inflammation factors were remarkably decreased in observation group compared with control group after treatment(P<0.05).The adverse reaction rate was 95.00%in observation group,which was slightly higher than 93.94%in control group,with no statistical difference(x2=0.039,P=0.842).Conclusion Modified Chaihu Longmu decoction in effective for stroke patients.
