肺段切除和肺叶切除治疗早期肺癌的疗效及术后并发症的比较

摘要
目的 探讨肺段切除和肺叶切除治疗早期肺癌的疗效及术后并发症的比较.方法 选取2020年1月～2021年1月我院收入早期肺癌患者102例,按照随机数字表法分为试验组(n=51)及对照组(n=51),试验组行肺段切除治疗,对照组行肺叶切除治疗,对比两组患者手术时间、术中出血量、住院时间差异,比较两组患者术前及术后1个月肺功能[1s用力呼气容积(FEV1)、用力肺活量(FVC)、每分钟最大通气量(MVV)]变化;对比术后并发症发生情况、1年无瘤生存期差异.结果 试验组患者术中出血量、住院时间均小于对照组,差异具有统计学意义(P＜0.05);两组手术时间比较,差异无统计学意义(P＞0.05);试验组患者治疗后FEV1、FVC、MVV等肺功能指标均小于治疗前,且试验组大于对照组,差异具有统计学意义(P＜0.05);两组患者术后并发症发生率比较,差异无统计学意义(P＞0.05);两组术后1年无瘤生存期比较,差异无统计学意义(P＞0.05).结论 早期肺癌患者应用肺段切除与肺叶切除均可延长生存时间,并发症少,安全可行,且肺段切除能够在保护患者肺功能方面更有效.
Abstract
Objective To compare the efficacy and postoperative complications of segmental and lobectomy in the treat-ment of early lung cancer.Methods From January 2020 to January 2021,102 patients with early-stage lung cancer in our hospital were selected and divided into the test group(n=51)and the control group(n=51)according to the random num-ber table method.The test group was treated with segmental lung resection and the control group was treated with lobectomy,The changes of pulmonary function[1 s forced expiratory volume(FEV1),forced vital capacity(FVC)and maximum ventila-tion per minute(MVV)]were compared between the two groups before and one month after operation;The incidence of post-operative complications and 1-year tumor free survival were compared.Results the bleeding volume and hospitalization time in the experimental group were significantly lower than those in the control group(P＜0.05);There was no significant difference in the operation time between the two groups(P＞0.05);After treatment,FEV1,FVC,MVV and other pulmonary function indexes in the test group were lower than those before treatment,and the difference was statistically significant(P＜0.05);There was no significant difference in the incidence of postoperative complications between the two groups(P＞0.05);There was no significant difference in the 1-year tumor free survival between the two groups(P＞0.05).Conclusion Both segmental resection and lobectomy can prolong the survival time of patients with early lung cancer,with fewer com-plications,safety and feasibility,and segmental resection can be more effective in protecting the lung function of patients.
