目的 研究自拟温阳通络汤治疗脑梗死恢复期患者对其血液流变学及神经功能的影响.方法 选取本院2020年5月至2021年9月期间115例脑梗死恢复期患者随机分组,对照组57例给予常规治疗,观察组58例联合自拟温阳通络汤治疗,对比两组患者临床疗效、血液流变学指标、血清学指标、神经功能和不良反应.结果 观察组治疗后,血浆黏度、全血黏度、纤维蛋白原水平均低于对照组(P＜0.05);观察组治疗后,血小板α-颗粒膜糖蛋白(GMP-140)、巨噬细胞集落刺激因子(M-CSF)水平均低于对照组(P＜0.05);观察组治疗后,S100B蛋白(S-100B)、神经元特异性烯醇化酶(NSE)、神经胶质纤维酸性蛋白(GFAP)水平均低于对照组(P＜0.05);观察组治疗总有效率高于对照组(Z=2.1634,P＜0.05);两组头晕、上腹饱胀、呕吐等不良反应发生率差异无统计学意义(P＞0.05).结论 自拟温阳通络汤治疗脑梗死恢复期患者效果确切,能够有效降低血浆黏度,保护神经功能,安全性高.
Abstract
Objective To study the effect of Wenyang Tongluo Decoction on hemorheology and neurological function in patients with cerebral infarction during convalescence.Methods A total of 115 patients with cerebral infarction during re-covery period in our hospital from May 2020 to September 2021 were randomly divided into the control group(n=57)and the observation group(n=58).The control group received conventional treatment,and the observation group received self-designed Wenyang Tongluo decoction(n=58).The clinical efficacy,blood rheology indexes,serological indexes,neurologi-cal function and adverse reactions were compared between the two groups.Results After treatment,the plasma viscosity,whole blood viscosity and fibrinogen in the observation group were lower than those in the control group(P＜0.05).After treatment,the levels of platelet α-granule membrane glycoprotein(GMP-140)and macrophage colony-stimulating factor(M-CSF)in the observation group were lower than those in the control group(P＜0.05).After treatment,the levels of S100B protein(S-100B),neuron-specific enolase(NSE)and glial fibrillary acidic protein(GFAP)in the observation group were lower than those in the control group(P＜0.05).The total effective rate of the observation group was higher than that of the control group(Z=2.1634,P＜0.05).There was no significant difference in the incidence of adverse reactions such as diz-ziness,epigastric distension and vomiting between the two groups(P＞0.05).Conclusion The self-designed Wenyang Tongluo decoction is effective in the treatment of patients with cerebral infarction in the convalescent period,which can ef-fectively reduce plasma viscosity,protect nerve function,and has high safety.
