治疗老年难治性心力衰竭并利尿剂抵抗应用大剂量呋塞米联合氨茶碱的价值

摘要
目的 观察在老年难治性心力衰竭并利尿剂抵抗患者的临床治疗中,实施大剂量呋塞米联合氨茶碱方案治疗,所取得的效果和价值.方法 本研究时段介于2020年3月～2021年12月,对我院70例老年难治性心力衰竭并利尿剂抵抗患者进行规范分组操作;对照组33例实施大剂量呋塞米治疗,观察组37例联合氨茶碱治疗;就两组临床疗效、心功能、心脏指数(CI)、N末端脑钠肽前体(NT-pro BNP)以及肾功能展开观察并对比.结果 (1)经对观察组临床疗效进行评估,结果为97.2％与对照组评估结果78.7％展开比较,观察组临床疗效居更高水平(P＜0.05).(2)实施治疗工作后,两组心功能均有显著变化;与对照组数据相比,观察组心功能改善幅度较大(P＜0.05).(3)实施治疗工作后,与对照组数据相比,观察组CI、NT-pro BNP以及24 h蛋白尿、Scr水平较低(P＜0.05).结论 大剂量呋塞米联合氨茶碱方案,在老年难治性心力衰竭并利尿剂抵抗的治疗中具有显著疗效,值得临床借鉴并推广.
Abstract
Objective To observe the effect and value of high-dose furosemide combined with aminophylline in the clin-ical treatment of elderly patients with refractory heart failure and diuretic resistance.Methods The study period was from March,2020 to December,2021.70 elderly patients with refractory heart failure and diuretic resistance were divided into groups according to the standard;33 cases in the control group were treated with high-dose furosemide,and 37 cases in the observation group were treated with aminophylline;The clinical efficacy,cardiac function,cardiac index(CI),N-terminal pro brain natriuretic peptide(NT Pro BNP)and renal function were observed and compared between the two groups.Results(1)After evaluating the clinical efficacy of the observation group,the result was 97.2％compared with 78.7％of the con-trol group,and the clinical efficacy of the observation group was at a higher level(P＜0.05).(2)After the treatment,the cardiac function of the two groups changed significantly;Compared with the data of the control group,the cardiac function of the observation group improved significantly(P＜0.05).(3)After the treatment,compared with the data of the control group,the levels of Ci,NT Pro BNP,24h proteinuria and SCR in the observation group were lower(P＜0.05).Conclusion High dose furosemide combined with aminophylline has a significant effect in the treatment of elderly refractory heart fail-ure with diuretic resistance,which is worthy of clinical reference and promotion.
