孕早期膳食多不饱和脂肪酸与出生结局的关系
The relationship between dietary polyunsaturated fatty acids during pregnancy and birth outcomes

摘要
目的 探讨孕早期膳食多不饱和脂肪酸与出生结局的关系.方法 选取2017年4月~2018年6月在我院产科门诊建卡并接受检测的孕早期妇女1813例.采用3天24小时(两个工作日,一个周末),调查孕妇孕早期膳食摄入情况.采用单因素Logistic回归模型分析母亲年龄、产次、孕期吸烟、入选时胎龄、孕前身体质量指数(body mass index,BMI)以及膳食摄入多不饱和脂肪酸与出生结局的关系,并将单因素有意义的变量纳入多因素回归模型,分析影响出生结局的独立危险因素.结果 1813名孕妇中,平均年龄(29.3±3.2)岁,平均产次为(1.8± 0.3)次,入选时平均胎龄(9.7±0.6)周,孕前BMI为(22.8±3.7)kg/m2.孕期吸烟者38(4.7%)例,早产儿113(6.2%)例,低出生体重儿101(5.6%)例,小于胎龄儿230(12.7%)例.多因素Logistic回归分析显示,与膳食摄入多不饱和脂肪酸最低剂量组相比(Q1),膳食摄入多不饱和脂肪酸最高剂量组(Q4)是早产[OR:0.84;(95%CI:0.71~0.92)]、低出生体重[OR:0.82(95%CI:0.55~0.89)]和小于胎龄儿[OR:0.91(95%CI:0.86~0.97)]的保护因素.结论 孕早期膳食摄入多不饱和脂肪酸可以降低早产、低出生体重和小于胎龄儿的发生风险.
Abstract
Objective To investigate the relationship between dietary polyunsaturated fatty acids during pregnancy and birth outcomes.Methods During April 2017 to June 2018,1813 early pregnant women in the maternity clinic of our hospi-tal were included.The dietary intake of polyunsaturated fatty acids(PUFA)in pregnant women was investigated by 24 hours(two working days and one weekend)diet for 3 days.Univariate logistic regression model was used to analyze the relation-ship between maternal age,parity,smoking during pregnancy,gestational age at admission,body mass index(BMI)before pregnancy and dietary intake of polyunsaturated fatty acids(PUFA)and birth outcomes.The variables showed significant in univariate Logistic regression model were introduced in multivariable Logistic regression model.Results Among the 1813 pregnant women,the average age was(29.3±3.2)years old,the average number of births was(1.8±0.3),and the average gestational age was(9.7±0.6)weeks,the pre-pregnancy BMI was(22.8±3.7)kg/m2.Among them,there were 38(4.7%)smokers,113(6.2%)premature infants,101(5.6%)low birth weight infants and 230(12.7%)small for gesta-tional age.Multivariate logistic regression analysis showed that compared with the lowest dose group of dietary PUFA(Q1),the highest dose group of dietary PUFA(Q4)was a protective factor for premature delivery[OR:0.84;(95%CI:0.71~0.92)],low birth weight[OR:0.82(95%CI:0.55~0.89)]and smaller than gestational age[OR:0.91(95%CI:0.86~0.97)].Conclusion Dietary intake of PUFA during early pregnancy may reduce the risk of premature delivery,low birth weight and small for gestational age.
