摘要
目的 探究曼月乐联合GnRH-a治疗子宫腺肌病效果及对子宫内膜厚度及月经量影响.方法 研究选取南阳市中心医院2017年1月～2019年12月进行保守治疗的58例子宫腺肌病患者为研究对象,根据治疗方法不同分曼月乐组和观察组(曼月乐联合GnRH-a治疗),每组各29例.观察两组患者子宫体积变化、月经出血量评分及不良反应.结果 治疗前观察组与曼月乐组子宫体积差异无统计学意义(P＞0.05),治疗结束、治疗结束后3个月观察组子宫体积小于曼月乐组,两组比较差异有统计学意义(P＜0.05);治疗前观察组与曼月乐组月经量评分差异无统计学意义(P＞0.05),治疗结束、治疗结束后3个月观察组月经量评分低于曼月乐组,观察组治疗结束后3个月月经量评分低于治疗结束,两组比较差异有统计学意义(P＜0.05);观察组不良反应发生率(41.4％)高于曼月乐组(27.6％),差异无统计学意义(P＞0.05).结论 曼月乐联合四针GnRH-a治疗能明显改善子宫腺肌病患者症状,缩小子宫体积,减少月经出血量,长期效果理想且安全性高,值得临床推广.
Abstract
Objective To explore the effect of Mirena combined with GnRH-a in the treatment of adenomyosis and the effect on endometrial thickness and menstrual flow.Methods Fifty-eight patients with adenomyosis who underwent conser-vative treatment in Nanyang Central Hospital from January 2017 to December 2019 were selected as the research objects.According to different treatment methods,they were divided into Mirena group and observation group(Mirena combined with GnRH-a Treatment),twenty-nine cases in each group.Observe the changes of uterine volume,menstrual bleeding scores and adverse reactions in the two groups.Results There was no significant difference in uterine volume between the observation group and the Mirena group before treatment(P＞0.05).The uterine volume in the observation group was smaller than that in the Mirena group after treatment and three months after treatment,and the uterine volume was slightly higher three months after treatment.At the end of treatment,the difference between the two groups was statistically significant(P＜0.05);there was no significant difference in menstrual volume scores between the observation group and the Mirena group before treat-ment(P＞0.05).At the end of the treatment,follow-up observations were made three months after treatment.The menstrual volume score of the group was lower than that of the Mirena group,and the menstrual volume score was lower than the end of treatment three months after treatment.The difference between the two groups was statistically significant(P＜0.05);The incidence of adverse reactions in the observation group(41.4％)was higher than that in the Mirena group(27.6％),and the difference was not statistically significant(P＞0.05).Conclusion Mirena combined with four-needle GnRH-a treat-ment can significantly improve the symptoms of patients with adenomyosis,reduce the size of the uterus,and reduce men-strual bleeding.The long-term effect is ideal and the safety is high.It is worthy of clinical promotion.
