摘要
目的 研究艾司奥美拉唑双途径给药联合奥曲肽静推治疗消化性溃疡合并上消化道出血患者的疗效评价.方法 选取2021年6月至2022年8月我院收治的60例消化性溃疡合并上消化道出血患者,采用数字表法分为对照组(n=30)和观察组(n=30).对照组采用奥曲肽静推治疗,观察组采用艾司奥美拉唑双途径给药联合奥曲肽静推治疗.比较两组临床疗效、平均止血时间、潜血转阴时间、出血量、输血量、治疗前后凝血功能指标[凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fib)]、血清a防御素(DEFA)、内脂素(Visfatin)、C反应蛋白(CRP)、肿瘤坏死因子-a(TNF-a)水平及不良反应.结果 观察组总有效率90.00％高于对照组66.67％(P＜0.05);与对照组比较,观察组平均止血时间、潜血转阴时间较短,出血量、输血量较低(P＜0.05);治疗后与对照组相比,观察组TT、Visfatin、PT、CRP、APTT、TNF-a较低,Fib、DEFA较高(P＜0.05);不良反应发生率组间比较,差异无统计学意义(P＞0.05).结论 艾司奥美拉唑双途径给药联合奥曲肽静推治疗消化性溃疡合并上消化道出血效果确切,可有效止血,减轻机体炎症反应,且安全性高.
Abstract
Objective To evaluate the efficacy of esomeprazole combined with octreotide in the treatment of patients with peptic ulcer complicated with upper gastrointestinal bleeding.Methods Sixty patients with peptic ulcer complicated with upper gastrointestinal bleeding admitted to our hospital from June 2021 to August 2022 were selected and divided into control group(n=30)and observation group(n=30)by digital table method.The control group was treated with octreotide intravenous infusion,and the observation group was treated with esomeprazole dual-route administration combined with oct-reotide intravenous infusion.The clinical efficacy,mean time of hemostasis,time of occtonegative conversion,blood loss,blood transfusion volume,coagulation function indexes before and after treatment[thrombin time(TT),prothrombin time(PT),activated partial thrombin time(APTT),fibrinogen(Fib)],serum α-defA,Visfatin,and C were compared between the two groups Reactive protein(CRP),tumor necrosis factor-α(TNF-α)levels and adverse reactions.Results The total effective rate of observation group was 90.00％higher than that of control group 66.67％(P＜0.05).Compared with the control group,the average hemostatic time and occult blood turning negative time in the observation group were shorter,and the blood loss and blood transfusion volume were lower(P＜0.05).After treatment,TT,Visfatin,PT,CRP,APTT and TNF-αin observation group were lower than those in control group,while Fib and DEFA were higher(P＜0.05).There was no signif-icant difference in the incidence of adverse reactions between groups(P＞0.05).Conclusion Esomeprazole dual route ad-ministration combined with octreotide static push is effective in the treatment of peptic ulcer complicated with upper diges-tive tract bleeding,which can effectively stop bleeding,reduce the inflammatory reaction of the body,and has high safety.
