抗环瓜氨酸肽抗体在类风湿关节炎诊断、疾病活动度、骨破坏发病机制中的研究进展

摘要
类风湿关节炎(RA)是一种炎症性疾病,可导致严重的关节炎症、滑膜增生、血管翳形成、软骨和骨骼破坏.抗环瓜氨酸肽(CCP)抗体不仅是RA诊断的重要血清学标志物,而且是预测RA疾病活动度及骨破坏进展的重要血清学标志物.由于进行性骨破坏,RA患者的生活质量急速恶化,因此通过早期诊断及干预抑制疾病进展和关节破坏已成为重要的临床目标.本综述就抗CCP抗体检测RA的特异性和灵敏性、抗CCP抗体与RA疾病活动及与骨破坏间的相关性等三个不同的方面进行探讨,以期为RA发病机制研究及诊疗提供新思路.
Abstract
Rheumatoid arthritis(RA)is an inflammatory disease that causes severe joint inflammation,synovial hyper-plasia,pannus formation,and cartilage and bone destruction.Anti-cyclic citrullinated peptide(CCP)antibody is not only an important serological marker for the diagnosis of RA,but also an important serological marker for predicting the disease ac-tivity and progression of bone destruction in RA.Due to progressive bone destruction,the quality of life of RA patients dete-riorates rapidly.Therefore,early diagnosis and intervention have become important clinical goals to inhibit disease progres-sion and joint destruction.In this review,the specificity and sensitivity of anti-CCP antibody in detecting RA,and the corre-lation between anti-CCP antibody and disease activity and bone destruction in RA were discussed,so as to provide new ideas for the pathogenesis,diagnosis and treatment of RA.
