摘要
目的 探讨在单纯乳腺导管原位癌、浸润性导管癌鉴别诊断中应用MRI-DWI的效用价值.方法 此次研究共选取乳腺癌患者92例,其中单纯乳腺导管原位癌、浸润性导管癌各46例.对其展开MRI(核磁共振成像)检查与DWI(扩散加权成像)检查,对检查结果展开观察比较.结果 经实验数据表明,DCIS患者无线状分布明显高于IDC患者,后者分支线状分布多于前者(P＜0.05);与IDC患者相比,DCIS患者簇环状/网格状清晰与病变信号均匀情况均相对较优(P＜0.05);两组最大直径、ADCmax无显著差异(P＞0.05).但IDC患者ADCmin明显低于DCIS患者,扩散系数下降率也相对较低(P＜0.05).ROC曲线中表明,联合检查的约登指数、敏感度与特异度相对较高,表明其具有极高的诊断价值.结论 在DCIS、IDC病症诊断中应用MRI-DWI技术手段能够有效提高诊断精确度,具有较高的应用价值.
Abstract
Objective To investigate the utility of MRI-DWI in the differential diagnosis of simple ductal carcinoma in situ and invasive ductal carcinoma of the breast.Methods A total of 92 patients with breast cancer were selected in this study,including 46 cases of simple ductal carcinoma in situ and 46 cases of invasive ductal carcinoma.MRI(Magnetic Reso-nance Imaging)and DWI(Diffusion Weighted Imaging)examinations were performed,and the results were observed and compared.Results The experimental data showed that the wireless distribution in DCIS patients was significantly higher than that in IDC patients,and the branch linear distribution in the latterwas higher than that in the former(P＜0.05);Compared with IDC patients,DCIS patients had better cluster ring/grid clarity and lesion signal uniformity(P＜0.05);There was no significant difference in maximum diameter and ADC max between the two groups(P＞0.05).However,the ADC min of IDC patients was significantly lower than that of DCIS patients,and the decrease rate of diffusion coefficient was relatively low(P＜0.05).The ROC curve shows that the Yoden index,sensitivity,and specificity of the combined examina-tion are relatively high,indicating that it has high diagnostic value.Conclusion The application of MRI-DWI in the diag-nosis of DCIS and IDC can effectively improve the diagnostic accuracy and has high application value.
