无创呼吸机联合左氧氟沙星治疗老年重症肺炎的疗效及对炎症因子的影响

摘要
目的 本研究旨在分析在左氧氟沙星治疗老年重症肺炎基础上使用无创呼吸机辅助治疗对疗效和炎症的影响.方法 获取2020年10月到2023年10月在本院治疗的老年重症肺炎患者114例为研究对象.根据随机数字表法,把研究对象随机分成对照组和研究组,各57例.两组均给予常规治疗,在此基础上对照组给予患者静脉滴注左氧氟沙星,研究组在对照组的基础上使用无创呼吸机进行辅助呼吸.比较两组临床疗效,比较两组治疗前后肺功能,比较两组治疗前后炎症因子水平.结果 研究组总有效率为96.49％高于对照组的总有效率为84.21％(P＜0.05);治疗后研究组FVC和FEV1水平高于对照组(P＜0.05);治疗后两组CRP、IL-6水平均下降,且对照组CRP、IL-6水平均高于研究组(P＜0.05);研究组治疗后不良反应发生率为3.51％,对照组治疗后不良反应发生率为7.02％,两组比较无明显差异(P＞0.05).结论 无创呼吸机辅助左氧氟沙星治疗老年重症肺炎有显著效果,能改善肺功能,缓解炎症反应.
Abstract
Objective To investigate the efficacy of non-invasive ventilation combined with levofloxacin in the treatment of severe pneumonia in the elderly and its impact on inflammatory factors.Methods One hundred and fourteen elderly pa-tients with severe pneumonia in our hospital from October 2020 to October 2023 were enrolled,and randomly assigned into two groups using random number table methods,each with 57 cases.All participants were subjected to routine treatment,based on this,control group added intravenous administration of levofloxacin,and study group received intravenous adminis-tration of levofloxacin and non-invasive ventilation.Then the clinical efficacy,pulmonary function,and inflammatory factors were compared between two groups before and after treatment.Results The total efficacy rate was 96.49％in study group,which was higher than 84.21％in control group(P＜0.05).The values of forced vital capacity(FVC)and forced expiratory volume in the first second(FEVI)were larger in study group than control group after treatment(P＜0.05).CRP and IL-6 levels decreased in both groups after treatment,and CRP and IL-6 levels in the control group were higher than those in the study group(P＜0.05).The incidence of post-treatment adverse reactions in the study group was 3.51％and 7.02％.There was no significant difference between the two groups(P＞0.05).Conclusion Application of non-invasive ventila-tion combined with levofloxacin in the treatment of severe pneumonia in the elderly has good clinical effect,which can effec-tively ameliorate the pulmonary function and attenuate inflammation response.
