含环丝氨酸对耐多药肺结核患者促进痰结核杆菌转阴的疗效分析

摘要
目的 分析含环丝氨酸对耐多药肺结核(MDR-TB)患者促进痰结核杆菌转阴、病灶吸收及病灶空洞闭合的疗效.方法 选取我院(2016年11月~2018年10月)收治的104例MDR-TB患者的临床资料进行分析,根据治疗方案不同分为两组.对照组采用世界卫生组织标准治疗方案治疗,观察组采用在标准治疗方案基础上联合含环丝氨酸的方案治疗,分析两组患者治疗后的临床疗效.结果 观察组3个月、6个月、9个月及12个月时痰结核杆菌转阴率均高于对照组(P<0.05).观察组治疗后IFN-y水平高于对照组,IL-4水平低于对照组(P<0.05).经过治疗,观察组病灶-X线片吸收情况优于对照组(P<0.05).观察组失眠焦虑的发生率高于对照组(P<0.05).两组不良反应发生率比较,未见统计学差异(P>0.05).结论 含环丝氨酸治疗MDR-TB可提高痰结核杆菌转阴率,促进病灶吸收,改善IFN-γ、IL-4的表达水平,但应注意的精神、神经方面副反应情况.
Abstract
Objective To analyze the effect of cycloserine on sputum tuberculosis,lesion absorption and cavity closure in patients with multi drug resistant pulmonary tuberculosis(MDR-TB).Methods the clinical data of 104 patients with MDR-TB admitted to our hospital from November 2016 to October 2018 were analyzed and divided into two groups according to different treatment schemes.The control group was treated with WHO standard treatment,and the observation group was treated with cycloserine combined with standard treatment.The clinical efficacy of the two groups was analyzed.Results The clinical data of 104 patients with MDR-TB admitted to our hospital(from November 2016 to October 2018)were ana-lyzed and divided into 2 groups according to different treatment plans.The control group was treated with WHO standard treatment regimen,and the observation group was treated with the combination of cycloserine containing regimen based on the standard treatment regimen.The clinical efficacy of the two groups after treatment was analyzed.There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Cycloserine in the treat-ment of MDR-TB can improve the negative conversion rate of Mycobacterium tuberculosis,promote the absorption of lesions,and improve the expression levels of IFN-γ and IL-4,but the mental and neurological side effects should be paid attention to.
