摘要

目的 通过检测miR-15a、miR-16在癫痫患者血清中表达变化,分析其与认知功能的关系.方法 选择2020年1月至2021年3月本院治疗的99例癫痫患者作为癫痫组,依据是否存在认知功能障碍分为认知功能正常组(32例)与认知障碍组(67例);认知障碍组进一步分为轻度组(41例),中度组(13例)与重度组(13例).同期本院健康体检的志愿者为对照组.比较各组血清miR-15a、miR-16表达水平差异,分析癫痫患者血清miR-15a、miR-16表达水平与蒙特利尔认知评估量表(MoCA)评分的相关性;多因素Logistic回归分析癫痫患者发生认知功能障碍的影响因素.结果 癫痫组血清miR-15a、miR-16表达水平及MoCA评分均低于对照组(P<0.05);相关性分析显示,癫痫患者血清miR-15a、miR-16表达水平与MoCA评分呈正相关(r=0.456、0.493,P<0.05);认知障碍组血清miR-15a、miR-16表达水平低于认知功能正常组(P<0.05);回归分析显示,病程、发作频率及miR-15a、miR-16 是癫痫患者发生认知功能障碍的影响因素(P<0.05);轻度组血清miR-15a、miR-16表达水平高于中度组、重度组,中度组血清miR-15a、miR-16表达水平高于重度组(P<0.05).结论 癫痫患者血清miR-15a、miR-16表达水平降低,是发生认知功能障碍的影响因素,且与认知功能障碍严重程度呈正相关.

Abstract

Objective By detecting the expression of miR-15a and miR-16 in the serum of epileptic patients,the rela-tionship between them and cognitive function was analyzed.Methods Ninety-nine patients with epilepsy treated in our hos-pital from January 2020 to March 2021 were selected as the epilepsy group,and were divided into the group with normal cognitive function(32 cases)and the group with cognitive impairment(67 cases)according to whether they had cognitive dysfunction.The cognitive impairment group was further divided into mild group(41 cases),moderate group(13 cases)and severe group(13 cases).During the same period,the healthy volunteers in our hospital were the control group.The expres-sion levels of serum miR-15a and miR-16 in all groups were compared,and the correlation between the expression levels of serum miR-15a and miR-16 and the score of Montreal Cognitive Assessment Scale(MoCA)in patients with epilepsy was an-alyzed.Multivariate logistic regression analysis was used to analyze the influencing factors of cognitive dysfunction in epilep-tic patients.Results The expression levels of miR-15a and miR-16 in serum and MoCA score in epilepsy group were lower than those in control group(P<0.05).Correlation analysis showed that the expression level of miR-15a and miR-16 in epi-leptic patients was positively correlated with MoCA score(r=0.456,0.493,P<0.05);The expression levels of miR-15a and miR-16 in cognitive impairment group were lower than those in normal cognitive function group(P<0.05).Regression analysis showed that the course of disease,seizure frequency and miR-15a and miR-16 were influencing factors of cognitive dysfunction in epilepsy patients(P<0.05).The expression levels of serum miR-15a and miR-16 in mild group were higher than those in moderate and severe group,and the expression levels of serum miR-15a and miR-16 in moderate group were higher than those in severe group(P<0.05).Conclusion The decreased expression levels of serum miR-15a and miR-16 in patients with epilepsy are the influencing factors for the occurrence of cognitive dysfunction,and are positively correlated with the severity of cognitive dysfunction.
