品管圈结合循证护理在急性缺血性脑卒中静脉溶栓治疗患者中的应用效果

摘要
目的 探讨品管圈结合循证护理在急性缺血性脑卒中静脉溶栓治疗患者中的应用效果.方法 选取2020年6月至2022年6月在我院行静脉溶栓治疗的102例急性缺血性脑卒中患者,根据数字盲选法分为观察组51例和对照组51例.对照组实施常规护理干预,观察组实施品管圈结合循证护理干预.比较两组患者到院至溶栓治疗时间(DNT)、DNT合格率及干预前后神经功能缺损情况和生活质量情况.结果 观察组DNT时间(51.37±10.56)min 短于对照组(65.29±10.75)min,DNT 合格率 90.20(46/51)高于对照组 64.71％(33/51),差异均有统计学意义(P＜0.05).观察组干预后NIHSS评分(13.37±3.94)分低于对照组(18.53±4.12)分,SF-36评分(79.28±5.42)分高于对照组(71.61±5.36)分,差异均有统计学意义(P＜0.05).结论 急性缺血性脑卒中患者静脉溶栓治疗应用品管圈结合循证护理干预能有效缩短DNT时间,提升DNT合格率,提高患者神经功能与生活质量,值得推广.
Abstract
Objective To discuss the effect of quality control circle combined with evidence-based nursing care in pa-tients with acute ischemic stroke undergoing intravenous thrombolysis.Methods One hundred and two patients with acute ischemic stroke undergoing intravenous thrombolysis in our hospital from June 2020 to June 2022 was enrolled,and assigned into two groups using random number table methods,each with 51 cases.Control group(n=51)received routine nursing in-tervention,whereas observation group(n=51)was given quality control circle combined with evidence-based nursing care.The the door-to-needle time(DNT),DNT qualification rate,neurological impairment and quality of life before and after in-tervention were compared between two groups.Results Observation group had shorter DNT[(51.37±10.56)min vs(65.29±10.75)min]and higher DNT qualification rate[90.20％(46/51)vs 64.71％(33/51)]than those of control group,with statistical difference(all P＜0.05),and observation group scored lower on National Institutes of Health Stroke Scale(NIHSS)[(13.37±3.94)vs(18.53±4.12)]and higher on Short Form 36-item(SF-36)[(79.28±5.42)vs(71.61±5.36)]compared to control group(all P＜0.05).Conclusion Application of quality control circle combined with evi-dence-based nursing care in patients with acute ischemic stroke undergoing intravenous thrombolysis can effectively shorten the DNT,and improve the DNT qualification rate,neurological impairment and quality of life in patients,which is worthy of recommendation.
