摘要
目的 探讨ABCDEF集束化策略对重症监护病房(intensive care unit,ICU)俯卧位通气患者的影响.方法 选取2020年5月~2022年5月我院ICU收治的106例急性呼吸窘迫综合征(acute respiration dysfunction syn-drome,ARDS)患者,随机分为观察组和对照组,每组53例,对照组给予ICU危重患者常规护理,观察组在对照组的基础上实行ABCDEF集束化策略,比较两组患者血气分析指标、使用机械通气时间和在ICU住院时间、发生谵妄的几率和谵妄持续的时间.结果 观察组患者血氧饱和度指数和血氧分压指数高于对照组,二氧化碳分压指数低于对照组(P<0.05);观察组患者使用机械通气时间和在ICU住院时间短于对照组(P<0.05);观察组患者发生谵妄的几率低于对照组,谵妄持续的时间短于对照组,差异有统计学意义(P<0.05).结论 ABCEDE集束化策略能够很好地改善ICU俯卧位通气患者的氧合状态,预防获得性谵妄的发生,提高患者的舒适度,促进患者的康复.
Abstract
Objective To discuss the effect of ABCDEF bundle on intensive care unit(ICU)patients undergoing prone position ventilation.Methods One hundred and six patients with acute respiration dysfunction syndrome(ARDS)in ICU of our hospital from May 2020 to May 2022 were enrolled,and randomly classified into groups,each with 53 cases.Control group received conventional nursing care,based on this,observation group received ABCDEF bundle.Then the blood gas a-nalysis indicators,length of mechanical ventilation and ICU stay,incidence rate of delirium and duration of delirium were compared between two groups.Results Compared with control group,observation group had an increase in oxygen satura-tion and partial pressure,and a decrease in partial pressure of carbon dioxide,with statistical difference(all P<0.05).Ob-servation group had decreased time to mechanical ventilation and ICU stay compared with control group,with statistical difference(all P<0.05).Patients in observation group had a lower incidence of delirium and a shorter duration of delirium than those of control group(all P<0.05).Conclusion Application of ABCDEF bundle in ICU patients undergoing prone position ventilation can effectively ameliorate the oxygenation status,prevent the occurrence of delirium,improve the comfort level,and promote the recovery of patients.
