摘要
目的 分析不同气道炎症表型支气管哮喘儿童的肺功能情况.方法 选择医院2020年1月至2022年1月诊治的支气管哮喘患儿90例,按照气道炎症表型分为嗜酸粒细胞型哮喘(EA)、中性粒细胞型哮喘(NA)、混合细胞型哮喘(MA)、寡细胞型哮喘(PA),例数分别是71例、10例、6例、3例.检测患者痰液中诱导痰嗜酸粒细胞及中性粒细胞比值,检测其肺功能.结果 EA、NA、MA、PA患者痰液中诱导痰嗜酸粒细胞及中性粒细胞比值差异比较有统计学意义(P＜0.05).EA、NA、MA、PA患者FEV1占预计值、FVC占预计值、FEV1/FVC实测值占预计值差异比较有统计学意义(P＜0.05).结论 不同气道炎症表型支气管哮喘患儿的肺功能存在差异,EA患儿肺功能差,PA患儿肺功能相对较好,建议实行个性化治疗.
Abstract
Objective To analyze the lung function in children with bronchial asthma with different airway inflammatory phenotypes.Methods Ninety children with bronchial asthma treated in the hospital from January 2020 to January 2022 were selected.Children were classified into eosinophilic asthma(EA,n=71),neutrophilic asthma(NA,n=10),mixedgran-ulocytic asthma(MA,n=6)and paucigranulocytic asthma(PA,n=3)according to airway inflammation phenotype.The ratio of eosinophils to neutrophils in the induced sputum of patients was calculated,and their lung function parameters were also measured.Results The ratio of eosinophil and neutrophil in induced sputum yielded statistical difference among EA,NA,MA and PA patients(P＜0.05).The forced expiratory volume in one second(FEV1),forced vital capacity(FVC)and the FEV1/FVC ratio expressed as percentage of predicted values demonstrated statistical difference among EA,NA,MA and PA patients(P＜0.05).Conclusion There are differences in lung function in children with different airway inflammatory phe-notypes of bronchial asthma,with poor lung function in children with CA and relatively good lung function in children with PA,for which individualized treatment is recommended.
