摘要
目的 探讨血清线粒体偶联因子-6(CF-6)、Toll样受体4(TLR4)及中性粒细胞表面CD64(nCD64)表达在新生儿败血症病情与预后中的应用.方法 选取我院2020年4月至2022年7月收治的97例新生儿败血症患儿为研究对象,根据小儿危重评分(PCIS)将患儿分为极危重组31例、危重组26例、非危重组40例,比较3组入院时PCIS评分、全身性感染相关性功能衰竭(SOFA)评分及血清CF-6、TLR4、nCD64水平,分析入院时血清各指标水平与PCIS评分、SOFA评分的相关性;对比不同预后患儿(入院时、治疗2周后、治疗4周后)血清各指标水平,分析各指标不同水平新生儿败血症预后不良危险度.结果 入院时3组SOFA评分及血清CF-6、TLR4、nCD64水平比较:极危重组>危重组>非危重组,PCIS评分比较:极危重组<危重组<非危重组(P<0.05);血清CF-6、TLR4、nCD64与SOFA评分呈正相关、与PCIS评分呈负相关(P<0.05);治疗2周、4周后预后良好患儿血清CF-6、TLR4、nCD64低于预后不良患儿,高水平预后不良危险度是低水平的2.772、2.217、2.449倍/6.125、3.404、3.995倍(P<0.05).结论 检测血清CF-6、TLR4、nCD64水平能有效诊断出新生儿败血症病情程度及评估预后状态,可运用于临床研究.
Abstract
Objective To investigate the application of serum mitochondrial coupling factor-6(CF-6),Toll-like receptor 4(TLR4)and neutrophil surface CD64(nCD64)expression in neonatal sepsis and prognosis.Methods A total of 97 chil-dren with neonatal sepsis admitted to our hospital from April 2020 to July 2022 were selected as research objects.According to the pediatric critical score(PCIS),the children were divided into extremely critical group(31 cases),critical group(26 cases)and non-critical group(40 cases).PCIS score,SOFA score and serum levels of CF-6,TLR4 and nCD64 were com-pared among the three groups on admission,and the correlation between serum levels of each index and PCIS score and SO-FA score was analyzed.The levels of serum indicators in children with different prognosis(at admission,after 2 weeks of treatment,after 4 weeks of treatment)were compared,and the risk of poor prognosis of neonatal sepsis at different levels of each indicator was analyzed.Results SOFA score and serum levels of CF-6,TLR4 and nCD64 were compared among the three groups at admission:critically ill group>critically ill group>non-critically ill group;PCIS score was compared with critically ill group<critically ill group<non-critically ill group(P<0.05);Serum CF-6,TLR4 and nCD64 were positively correlated with SOFA score and negatively correlated with PCIS score(P<0.05).After 2 weeks and 4 weeks of treatment,the serum CF-6,TLR4 and nCD64 in the children with good prognosis were lower than those in the children with poor prog-nosis,and the risk of high level poor prognosis was 2.772,2.217,2.449 times of that in the children with low level(6.125,3.404,3.995 times of that in the children with low prognosis(P<0.05).Conclusion Detection of serum CF-6,TLR4,nCD64 levels can effectively diagnose the severity of neonatal sepsis and evaluate the prognosis,which can be used in clini-cal research.
