摘要
以往静脉溶栓有着严格的时间窗限制,醒后卒中(wake up stroke)或者发病时间不明的急性缺血性脑卒中(acute ischemic stroke AIS)患者因错过静脉溶栓时间窗而错失再灌注治疗的机会.随着多模影像学技术的发展,组织窗概念的提出为该部分超窗患者争取到了再灌注治疗的机会.本文就组织窗评估方案中的DWI-FLAIR不匹配、缺血低灌注-核心梗死不匹配、临床症状-核心梗死不匹配等方案的最新研究进展进行综述,为临床一线中超时间窗患者的再灌注治疗工作提供部分参考.
Abstract
In the past,intravenous thrombolysis has strict time Windows.ischemic stroke patients who wake up stroke or acute ischemic stroke AIS with unknown onset may miss additional opportunities for reperfusion treatment because of missed time Windows of intravenous thrombolysis.With the development of multi-mode imaging techniques,the concept of tissue window provides the opportunity for reperfusion therapy for patients with hyperwindow.In this paper,the latest research pro-gress of DWI-FLAIR mismatch,ischemia-hypoperfusion-core infarction mismatch and clinical symptom-core infarction mis-match in the tissue window evaluation scheme is reviewed,which provides a partial reference for intravenous thrombolysis beyond the time window in clinical first-line.
