摘要

目的 探究围术期血清金属蛋白酶-13(MMP-13)、胰岛素样生长因子-1(1GF-1)、C反应蛋白(CRP)水平变化与膝关节骨性关节炎(KOA)患者术后疾病转归的相关性及临床意义.方法 选取2020年2月-2023年2月我院收治的97例KOA患者作为研究对象,均行关节镜手术治疗,根据术后1个月恢复情况将其分为良好组(n=71)和一般组(n=26).比较两组术后1d、3d、5d、7d血清MMP-13、IGF-1、CRP水平及疼痛视觉模拟评分(VAS)、奎森功能演算(Lequesne)指数,采用Pearson分析相关性,采用受试者工作特征曲线(ROC)分析术后3d的预测价值.结果 组间分析:良好组术后1～7d MMP-13、CRP、VAS、Lequesne指数低于一般组,IGF-1高于一般组(P＜0.05);组内分析:两组术后1～7d血清MMP-13、CRP、VAS、Lequesne指数呈下降趋势,IGF-1呈上升趋势(P＜0.05);术后3d血清MMP-13、CRP与VAS呈正相关;IGF-1水平与VAS、Lequesne指数呈负相关(P＜0.05);术后3d血清MMP-13、IGF-1、CRP联合预测的AUC为0.765.结论 血清MMP-13、IGF-1、CRP与KOA患者病情及预后密切相关,可作为临床诊断疾病的重要依据.

Abstract

Objective To investigate the correlation and clinical significance of perioperative serum levels of metallo-proteinase-13(MMP-13),insulin-like growth factor-1(IGF-1)and C-reactive protein(CRP)with postoperative disease outcome in patients with knee osteoarthritis(KOA).Methods A total of 97 patients with KOA admitted to our hospital from February,2020 to February,2023 were selected as the research objects.All patients underwent arthroscopic surgery and were divided into a good group(n=71)and a general group(n=26)according to their recovery at 1 month after surgery.The serum levels of MMP-13,IGF-1,CRP,visual analogue scale(VAS)and Lequesne score were compared between the two groups at 1,3,5 and 7 days after operation.Pearson correlation analysis was used to analyze the correlation,and receiver operating characteristic curve(ROC)was used to analyze the predictive value of 3d after surgery.Results The levels of MMP-13,CRP,VAS and Lequesne index in the good group were lower than those in the general group,and the level of IGF-1 was higher than that in the general group(P＜0.05).Intra-group analysis:the serum MMP-13,CRP,VAS and Lequesne index of the two groups showed a downward trend,and IGF-1 showed an upward trend from 1 to 7 days after operation(P＜0.05).Serum MMP-13 and CRP were positively correlated with VAS at 3 days after operation.IGF-1 level was negatively correlated with VAS and Lequesne index(P＜0.05).The AUC of combined prediction of serum MMP-13,IGF-1 and CRP on postoperative day 3 was 0.765.Conclusion Serum MMP-13,IGF-1 and CRP are closely related to the condition and prognosis of KOA patients,which can be used as an important basis for clinical diagnosis of disease.
