Objective To analyze the image quality and clinical application value of magnetic resonance small field voxel-incoherence motion diffusion weighted imaging(rFOV IVIM-DWI)in the diagnosis of rectal cancer.Methods A to-tal of 100 patients with rectal cancer who received treatment in our hospital from January 2022 to January 2023 were select-ed as the study objects,and all of them underwent routine field voxel-indiscrete motion diffusion-weighted imaging(fFOV IVIM-DWI)and rFOV IVIM-DWI.Compare the subjective evaluation results of lesion deformation and lesion clarity of IVI-em-DWI images by two physicians,and the parameters of the two groups of magnetic resonance rFOV Iviem-DWI[apparent diffusion coefficient(ADC),true diffusion coefficient(D),false diffusion coefficient(D*),perfusion fraction(f)].ROC was used to analyze the value of rFOV IVIim-DWI parameters in the diagnosis of rectal cancer in stage Ⅲ and stage Ⅳ.Pathological biopsy results were taken as the"gold standard"and consistency was used to analyze the value of rFOV IVIim-DWI parameters in the diagnosis of rectal cancer in stage Ⅲ and stage Ⅳ.Results Both doctors believed that the degree of image deformation of rFOV IVIim-DWI sequence was lower than that of fFOV Iviim-DWI sequence,but the clarity of DWI and ADC was higher than that of fFOV Iviim-DWI sequence(P<0.05).ADC,D and f in stage Ⅲ and stage Ⅳ were lower than those in stage Ⅰ and stage Ⅱ groups(P<0.05),and there was no statistical significance in D*between two groups(P>0.05).ROC analysis proved that rFOV IVIM-DWI parameters could be used to diagnose rectal cancer in stage Ⅲ and stage Ⅳ.The areas under the curve were 0.921,0.907 and 0.913,respectively,and P<0.05.ADC≤0.895 x 10-3mm2/s,D≤0.868 x 10-3mm2/s,and f≤25.59 were defined as colorectal cancer in stage Ⅲ and stage Ⅳ.Consistency analysis confirmed that the accuracy,sensitivity,specificity,positive predictive value,negative predictive value and Kappa value were 94.00%,94.37%,93.10%,97.10%,84.38%,and 0.857 in the diagnosis of stage Ⅲ and stage Ⅳ rectal cancer by rFOV IVIM-DWI.Conclusion Magnetic resonance rFOV IVIim-DWI technology has higher image quality,can significantly im-prove the resolution of the image under the conventional field of view,and reduce deformation,and rFOV Iviim-DWI param-eters are of high value in differentiating the stages of rectal cancer.The magnetic resonance rFOV Iviim-DWI technology is used in the diagnosis of rectal cancer in stage Ⅲ and stage Ⅳ.It can further improve the sensitivity and accuracy of diag-nostic results,reduce the occurrence of missed diagnosis and misdiagnosis,provide a reliable basis for clinical diagnosis and treatment,and have high promotion value.
