乳酸杆菌联合苦参凝胶对女性宫颈高危型HPV感染患者阴道微生态及微炎症指标的影响

摘要
目的 探讨乳酸杆菌联合苦参凝胶对女性宫颈高危型HPV感染患者阴道微生态及微炎症指标的影响.方法 选取2021.2-2023.2于我院就诊的宫颈高危型HPV感染患者76例,采用随机数字表法分为研究组和对照组,各38例.对照组采用苦参凝胶治疗,研究组在对照组基础上联合应用乳酸杆菌治疗.观察并比较两组治疗后的阴道微生态、阴道菌群结构及多样性、阴道微炎症指标和不良反应发生率.结果 治疗后研究组转阴率92.10％高于对照组73.68％,差异有统计学意义(P＜0.05);治疗后两组阴道PH值、阴道乳酸杆菌异常比例、白带清洁度异常比例均较治疗前低,且研究组低于对照组(P＜0.05);治疗后两组阴道菌群密集度Ⅱ～Ⅲ级比例、菌群多样性Ⅱ～Ⅲ级比例均较治疗前升高;且研究组阴道菌群密集度Ⅱ～Ⅲ级比例、菌群多样性Ⅱ～Ⅲ级比例高于对照组(P＜0.05);治疗后两组血清中白细胞介素IL-2、IL-4指标均较治疗前降低,IL-12指标较治疗前升高,且研究组IL-2、IL-4低于对照组,IL-12高于对照组(P＜0.05);两组不良反应发生率比较无显著差异(P＞0.05).结论 乳酸杆菌联合苦参凝胶治疗可改善女性宫颈高危型HPV感染患者阴道微生态失衡,降低微炎症因子水平,治疗效果满意.
Abstract
Objective To discuss the effect of live lactobacillus capsule for vaginal use combined with Kushen gel on vaginal microecology and micro inflammation indicators in women with cervical high-risk human papilloma virus(HPV)in-fection.Methods Seventy-six patients with cervical high-risk HPV infection who attended our hospital from February 2021 to February 2023 were enrolled,and classified into two groups using the random number table method,each with 38 ca-ses.Control group was given Kushen gel,based on this,study group added live lactobacillus capsule for vaginal use.Then the vaginal microecology,vaginal microflora structure and diversity,vaginal microinflammation index and the incidence of adverse reactions were observed and compared between two groups.Results The negative conversion rate was 92.10％in study group,which was higher than 73.68％in control group(P＜0.05).The vaginal pH value,the proportion of vaginal lactobacilli imbalance,and the proportion of abnormal leucorrhoea cleanliness showed a decrease in both groups after treat-ment,and the decrease was more notable in study group than in control group(P＜0.05).After treatment,the proportion of grade Ⅱ to Ⅲ bacterial density and the proportion of grade Ⅱ to Ⅲ bacterial diversity demonstrated an increase in both groups,and the increase was more remarkable in study group than in control group(P＜0.05).A reduction in serum levels of interleukin(IL)-2 and IL-4 as well as an increase in IL-12 were observed in two groups after treatment,and both changes were more significant in study group than in control group(P＜0.05).No statistical difference was reported in adverse reac-tion rate between two groups(P＞0.05).Conclusion Application of live lactobacillus capsule for vaginal use combined with Kushen gel in women with cervical high-risk HPV infection can effectively ameliorate the imbalance of vaginal micro-ecology and attenuate the micro-inflammation response and improve the treatment satisfaction.
