沙库巴曲缬沙坦联合急诊PCI治疗急性心肌梗死合并心功能不全的效果研究

摘要
目的 研究在急性心肌梗死合并心功能不全患者中使用沙库巴曲缬沙坦联合急诊PCI的疗效.方法 选取我院2022年2月至2023年12月期间120例收治的急性心肌梗死合并心功能不全患者,随机分为对照组与观察组,每组60例,两组均行急诊PCI治疗,对照组术后给予马来酸依那普利,观察组术后给予沙库巴曲缬沙坦;治疗周期均为3个月.比较两组治疗前后的左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、可溶性生长刺激表达基因2蛋白(sST2)、N末端脑钠肽前体(NT-proBNP)和不良反应情况.结果 治疗后,观察组的LVEF高于对照组,LVEDD低于对照组(P＜0.05).治疗后,观察组的sST2和NT-ProBNP水平低于对照组(P＜0.05).治疗期间,观察组和对照组不良反应发生率分别为15.00％(9/60),13.33％(8/60),无统计学差异(P＞0.05).结论 急性心肌梗死合并心功能不全的临床治疗选用沙库巴曲缬沙坦联合急诊PCI能有效缓解心肌损伤,改善心功能,安全性较高.
Abstract
Objective To investigate the effect of sacubitril/valsartan combined with primary percutaneous coronary in-tervention(PCI)in the treatment of acute myocardial infarction and cardiac insufficiency.Methods One hundred and twen-ty patients with acute myocardial infarction and cardiac insufficiency admitted to our hospital from February 2022 to Decem-ber 2023 were selected,and randomly grouped,each with 60 cases.All patients were subjected to primary PCI.After surger-y,control group was given oral administration of enalapril maleate tablets,while observation group received sacubitril/valsar-tan.Treatment in both groups lasted for 3 months.Then the left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),soluble suppression of tumorigenicity 2(sST2),N-terminal pro-brain natriuretic peptide(NT-proBNP),and adverse effects were compared between groups.Results After treatment,observation group reported larger LVEF,smaller LVEDD,and lower concentrations of NT-proBNP and sST2 compared to control group,with statistical differ-ence(all P＜0.05).During treatment,the adverse reaction rate was 15.00％(9/60)in observation group and 13.33％(8/60)in control group,with no statistical difference(P＞0.05).Conclusion Application of sacubitril/valsartan combined with primary PCI for patients with acute myocardial infarction and cardiac insufficiency can effectively alleviate myocardial injury and improve cardiac function,and has a good safety profile.
