摘要
目的 分析骨质疏松性脊柱骨折(Osteoporotic spinal fracture,OVCF)患者使用椎体成形术(percutaneous vertebro plasty,PVP)的效果.方法 随机数字表法均分南阳市第一人民医院治疗的62例OVCF患者(2021年5月-2022年5月),A组(常规治疗)和B组(PVP),对比两组效果.结果 B组治疗有效率93.54％、并发症发生率3.23％优于 A 组的 74.19％、19.35％(x2=4.292、4.026,P=0.038、0.044);术后 3 月 B 组腰椎 JOA 评分[主观症状(6.78±1.62)分、临床体征(4.85±0.54)分、日常生活受限度(10.23±2.14)分、总分(23.59±1.26)分]、颈椎JOA评分[运动(6.35±0.64)分、感觉(4.51±0.86)分、膀胱功能(1.97±0.52)分、总分(13.58±2.46)分]、生活质量评分[躯体功能(84.62±4.23)分、社会功能(85.23±4.67)分、情绪功能(85.41±3.64)分、活力(84.53±3.97)分、生理功能(84.57±3.62)分、心理功能(85.32±3.67)分、精神状态(84.79±5.23)分]、Barthel评分[洗澡修饰(6.69±1.53)分、转移床椅(6.78±1.62)分、控制大小便(6.35±1.64)分、上下楼梯(6.24±1.49)分、穿衣进食(6.52±1.73)分、上厕所(6.48±1.56)分、总分(83.25±5.14)分]、椎体(前缘、中部、后缘)高度(26.65±4.34)mm、(27.23±3.46)mm、(29.84±4.26)mm 高于 A 组(t=5.459、5.568、3.485,P 均=0.000),B 组住院时间(18.21±3.14)d优于A组(P＜0.05).结论 OVCF患者使用PVP治疗椎体功能恢复效果良好.
Abstract
Objective To analyze the effect of percutaneous vertebro plasty(PVP)in patients with Osteoporotic spinal fracture(OVCF).Methods 62 patients with OVCF treated in Nanyang First People's Hospital from May 2021 to May 2022 were randomly divided into group A(conventional treatment)and group B(PVP),and the effects of the two groups were compared.Results The effective rate and complication rate of group B were 93.54％and 3.23％,which were better than those of group A(74.19％and 19.35％)(x2=4.292,4.026,P=0.038,0.044).Three months after surgery,J0A score of group B lumbar spine[subjective symptoms(6.78±1.62)points,clinical signs(4.85±0.54)points,tolerance limit of daily life(10.23±2.14)points,total score(23.59±1.26)points],cervical spine JOA score[movement(6.35±0.64)points,sensation(4.51±0.8)points 6)score,bladder function(1.97±0.52)score,total score(13.58±2.46)score],quality of life score[physical function(84.62±4.23)score,social function(85.23±4.67)score,emotional function(85.41±3.64)score,vitality(84.53±3.97)score,physiological function(13.58±2.46)score 84.57±3.62)points,psychological func-tion(85.32±3.67)points,mental state(84.79±5.23)points],Barthel score[bath modification(6.69±1.53)points,transfer bed chair(6.78±1.62)points,control of stool and urine(6.35±1.64)points,up and down stairs(6.2 4±1.49)points,dressing and eating(6.52±1.73)points,toilet(6.48±1.56)points,total score(83.25±5.14)points],vertebral body(leading edge,middle edge,posterior edge)height(26.65±4.34)mm,(27.23±3.46)mm,(29.84±4.26)mm high-er than A Group B(t=5.459,5.568,3.485,P=0.000),group B(18.21±3.14)d was better than group A(P＜0.05).Conclusion The treatment of OVCF with PVP has a good effect on the recovery of vertebral function.
