急性脑梗死患者"血清同型半胱氨酸、小而低密度脂蛋白、低密度脂蛋白"水平变化及其临床意义分析

摘要
目的 探讨急性脑梗死(ACI)患者血清同型半胱氨酸(Hcy)、小而低密度脂蛋白(sdLDL-C)、低密度脂蛋白(LDL)水平变化及其临床意义.方法 回顾性选取2021年3月-2023年3月我院收治的ACI患者180例,纳入观察组,根据2∶1选例原则,另选取同期、同年龄段无ACI者90例,纳入对照组.比较2组及不同病情程度患者血清Hcy、sdLDL-C、LDL水平,分析血清各指标水平与ACI病情程度的相关性及联合诊断价值.结果 观察组血清Hcy、sdLDL-C、LDL水平高于对照组(P＜0.05);不同病情程度患者血清Hcy、sdLDL-C、LDL水平比较:轻度＜中度＜重度(P＜0.05);血清Hcy、sdLDL-C、LDL水平与ACI患者病情严重程度均呈正相关,且联合诊断ACI的AUC为0.897,最佳预测敏感度、特异度分别为91.11％、87.78％,约登指数为0.789.结论 Hey、sdLDL-C、LDL在ACI患者血清中呈高表达,其水平与病情严重程度均呈正相关,联合检测对ACI具有一定诊断价值,可作为临床诊断疾病、评估病情的辅助指标.
Abstract
Objective To investigate the changes of serum homocysteine(Hcy),small low-density lipoprotein(sdLDL-C)and low-density lipoprotein(LDL)levels and their clinical significance in patients with acute cerebral infarction(ACI).Methods A total of 180 patients with ACI admitted to our hospital from March 2021 to March 2023 were retrospectively selected to be included in the observation group,and another 90 cases of those without ACI during the same period and in the same age group were selected to be included in the control group according to the principle of 2:1 case selection.Serum levels of Hey,sdLDL-C and LDL were compared between the two groups and patients with different disease degrees,and the correlation between serum levels of each index and the degree of ACI disease and the combined diagnostic value were ana-lyzed.Results The serum levels of Hey,sdLDL-C and LDL in observation group were higher than those in control group(P＜0.05).The levels of serum Hey,sdLDL-C and LDL in patients with different disease degrees were compared as follows:mild＜moderate＜severe(P＜0.05);Serum Hey,sdLDL-C and LDL levels were positively correlated with the severity of ACI patients,and the AUC for combined diagnosis of ACI was 0.897,the best predictive sensitivity and specificity were 91.11％and 87.78％,respectively,and the Yoden index was 0.789.Conclusion Hey,sdLDL-C and LDL are highly expressed in the serum of ACI patients,and their levels are positively correlated with the severity of the disease.Combined detection has certain diagnostic value for ACI,and can be used as an auxiliary index for clinical diagnosis and evaluation of the disease.
