麝香保心丸联合阿司匹林、氯吡格雷治疗急性心肌梗死合并糖尿病患者的疗效观察

摘要
目的:麝香保心丸联合阿司匹林、氯吡格雷治疗急性心肌梗死合并糖尿病患者的临床疗效观察.方法:采用随机数字表法将2013年12月-2014年12月收治的76例接受急诊PCI的急性心肌梗死合并糖尿病患者分为观察组与对照组,每组38例.两组患者在给予正规降糖治疗基础上,对照组给予拜阿司匹林、氯吡格雷治疗,观察组给予麝香保心丸联合拜阿司匹林片、氯吡格雷片治疗.观察两组患者的临床疗效.结果:观察组冠脉血流TIMI2级和3级患者占100％,较对照组97.37％明显提高,差异有统计学意义(P＜0.05);在空腹血糖、餐后2h血糖水平方面,两组比较差异无统计学意义(P＞0.05);治疗后,两组患者心率及血肌酐水平均明显降低(P＜0.05),但观察组改善程度优于对照组,差异有统计学意义(P＜0.05);与对照组比较,观察组患者2个月内不良事件的发生率明显降低,差异有统计学意义(P＜0.05).结论:麝香保心丸联合双联抗血小板治疗急性心肌梗死合并糖尿病患者可有效提高冠脉血流灌注,降低心血管不良事件发生,改善心功能,且对血糖无显著影响.
Abstract
Objective:To observe the clinical effect of Shexiang Baoxin Pill combined with aspirin and clopidogrel on acute myocardial infarction patients with diabetes mellitus.Methods:76 cases of patients with acute myocardial infarction combined diabetes mellitus that came to our hospital for treatment at the time from December 2012 to December 2014 were divided into two observation group and control group with 38 cases in each group.Two groups were given formal hypoglycemic therapy.Besides,the control group was given aspirin and clopidogrel treatment and the observation group Shexiang Baoxin Pill combined with aspirin and clopidogrel.We observed the clinical efficacy of two groups.Results:Observation group's coronary blood flow TIMI 2 and 3 patients (100％) increased significantly and were higher than 97.37％ of the control group and the difference was statistically significant (P ＜ 0.05).As for the fasting blood glucose and 2 h postprandial blood glucose level,the two groups had no statistical significance (P ＞ 0.05).After treatment,two groups' heart rate and serum creatinine levels were significantly lower(P ＜ 0.05) but the improvement of observation group was better than that of control group and the difference was statistically significant(P ＜0.05).Compared with the control group,the observation group had a significantly lower incidence of adverse events within 2 months and the difference was statistically significant (P ＜ 0.05).Conclusion:Shexiang Baoxin Pill combined with aspirin and clopidogrel in patients with acute myocardial infarction and diabetes mellitus can effectively increase coronary blood flow perfusion,reduce cardiovascular adverse events and improve heart function and it had no significant effect on blood sugar.
