摘要
肾结石是一种常见和多发的泌尿系结石,临床上主要表现为尿频、尿急、尿痛、血尿、腰痛等症状.西医治疗多采取药物及手术等方式,有一定的局限性.周恩超教授应用中医药治疗肾脏疾病的临证经验丰富,对肾结石的病因病机和遣方用药具有独特的见解.肾结石可归属于中医"淋证""腰痛"等范畴,多因外邪、情志、饮食及房劳所伤.周恩超教授认为肾结石的主要病机为肾虚,膀胱气化功能不利,湿热蕴结,气滞血瘀,津液日久凝聚为石,阻碍水液运行,气血运行不畅,进一步导致脾肾亏虚,气阴耗伤,加重肾结石进展.临证应分别从清热利湿、通淋排石,行气活血、化瘀排石,补益脾肾、滋养肾阴等方面辨证施治,虚实结合,标本兼顾,同时注重日常防护,临床疗效显著,并验案两则加以佐证.该文旨在详细阐述周恩超教授治疗肾结石的理论依据及临证经验,以期为临床治疗肾结石提供新的治疗思路及方案.
Abstract
Kidney stone is a kind of common and frequently occurring urinary calculi.The main clinical manifestations are frequent urination,urgency,pain,hematuria,low back pain and other symptoms.Western medicine usually adopts medicine and surgery,which has certain limitations.Professor ZHOU Enchao has rich clinical experience in the treatment of kidney diseases with traditional Chinese medicine,and has unique views on the etiology and pathogenesis of kidney stones and prescription medi-cation.In traditional Chinese medicine,kidney stones can be classified into the categories of"stranguria"and"low back pain",which are mostly injured by external pathogens,emotions,diet and house labor.Professor ZHOU Enchao believes that the main pathogenesis of kidney stones is kidney deficiency,unfavorable bladder Qi movement,accumulation of dampness and heat,Qi stag-nation and blood stasis,prolonged accumulation of body fluid into stones,hindering the movement of water and blood,resulting in poor circulation of Qi and blood,further leading to spleen and kidney deficiency,Qi and Yin depletion,and worsening the progres-sion of kidney stones.The clinical treatment should be based on differentiation of symptoms and signs,such as clearing heat and dampness,clearing the flow of fluids and expelling stones,promoting Qi and blood circulation,resolving blood stasis and expelling stones,tonifying spleen and kidney and nourishing kidney Yin.The combination of deficiency and excess should take into account both specimens,while paying attention to daily protection.The clinical efficacy is significant,and two proven cases should be used as evidence.This article aims to elaborate on Professor ZHOU Enchao's theoretical basis and clinical experience in treating kidney stones,in order to provide new treatment ideas and plans for clinical treatment of kidney stones.
