通过HIF-1α/NLRP3信号通路探讨血府逐瘀胶囊治疗急性脑梗死的作用机制

目的 该研究旨在通过HIF-1α/NLRP3信号通路探讨血府逐瘀胶囊治疗急性脑梗死的作用机制,以期为临床防治脑梗死提供新的依据.方法 分析2022年12月-2023年12月于医院收治的62例急性脑梗死患者为研究对象,随机分为研究组(给予血府逐瘀胶囊治疗)和对照组(给予阿替普酶治疗)各31例,统计两组疗效及不良反应,对比治疗前及治疗后的中医证候积分、血流速度、HIF-1α、NLRP3、caspase-1水平及血液流变学指标.结果 研究组和对照组治疗有效率分别为96.77%(30/31)和77.42%(24/31),两组比较差异具有统计学意义(P<0.05).两组治疗后半身不遂、口角歪斜、吞咽困难、言语不利、头晕恶心、口唇紫暗、肌肤甲错、气短乏力评分均明显升高(P<0.05),且研究组治疗后Vs、Vd、Vm水平明显高于对照组(P<0.05).两组治疗后Vs、Vd、Vm水平均明显升高(P<0.05),且研究组治疗后Vs、Vd、Vm水平明显高于对照组(P<0.05).两组治疗后HIF-1α、NLRP3、caspase-1水平均明显降低(P<0.05),且研究组治疗后HIF-1α、NLRP3、caspase-1水平明显低于对照组(P<0.05).两组治疗后FIB、HSV、PSV水平均明显降低(P<0.05),且研究组治疗后FIB、HSV、PSV水平明显低于对照组(P<0.05).治疗期间两组均未出现不良反应.结论 血府逐瘀胶囊能够通过HIF-1α/NLRP3信号通路发挥治疗急性脑梗死的作用.
Mechanism of Xuefu Zhuyu Capsule(血府逐瘀胶囊)Treating Acute Cerebral Infarction through HIF-1α/NLRP3 Signaling Pathway
Objective To investigate the mechanism of Xuefu Zhuyu Capsule(血府逐瘀胶囊)in the treatment of acute cerebral infarction through hypoxia-inducible factor 1α(HIF-1α)/NOD-like receptor thermal protein domain associated protein 3(NLRP3)signaling pathway,in order to provide a new basis for clinical prevention and treatment of cerebral infarction.Methods A total of 62 patients with acute cerebral infarction admitted to the hospital from December 2022 to December 2023 were analyzed as research objects.They were randomly divided into the study group(Xuefu Zhuyu Capsule)and the control group(talteplase),with 31 cases in each group.The efficacy and adverse reactions of the two groups were analyzed.The TCM syndrome scores,blood flow velocity,the levels of HIF-1α,NLRP3 and caspase-1 and hemorheology indexes were compared before and after treatment.Results The effective rate of the study group and the control group was 96.77%(30/31)and 77.42%(24/31),respectively,and the difference between the two groups was statistically significant(P<0.05).The scores of hemiplegia,angle skew,dysphagia,speech impairment,dizziness and nausea,dark lips,skin nail disorder and shortness of breath were significantly increased in both groups after treatment(P<0.05),and the values of peak systolic blood flow velocity(Vs),end diastolic blood flow velocity(Vd)and mean blood flow velocity(Vm)in the study group were significantly higher than those in the control group after treatment(P<0.05).After treatment,the values of Vs,Vd and Vm in both groups were significantly increased(P<0.05),and the values of Vs,Vd and Vm in the study group were significantly higher than those in the control group(P<0.05).The levels of HIF-1α,NLRP3 and caspase-1 in both groups were significantly decreased after treatment(P<0.05),and the levels of HIF-1α,NLRP3 and caspase-1 in the study group were significantly lower than those in the control group after treatment(P<0.05).The levels of fibrinogen(FIB),whole blood high shear viscosity(HSV)and plasma viscosity(PSV)in both groups were significantly decreased after treatment(P<0.05),and the levels of FIB,HSV and PSV in the study group were significantly lower than those in the control group after treatment(P<0.05).No adverse reactions occurred in both groups during treatment.Conclusion Xuefu Zhuyu Capsule can play a role in treating acute cerebral infarction through HIF-1α/NLRP3 signaling pathway.
