摘要
目的 分析姜桂益瘿汤辅助西医治疗脾肾阳虚型桥本甲状腺炎的效果.方法 选取145例秦皇岛市中医医院收治的桥本甲状腺炎患者,分为对照组(72例)和研究组(73例),分组方法为随机数字表法,选例时间设置在2022年2月-2023年4月.对照组采用西医治疗,研究组在对照组的基础上采用姜桂益瘿汤治疗,两组患者连续治疗12周.比较两组治疗后临床疗效及治疗期间不良反应发生情况,中医症状及体征积分、血清促甲状腺激素(TSH)、甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)水平、甲状腺体积、峡部厚度(治疗前后).结果 治疗后,研究组总有效率高于对照组;治疗后与治疗前比较,两组中医症状及体征(颈部梗塞、颈部肿大、食少纳呆、大便秘结、面目水肿、神疲乏力、怕冷、体质量指数增加)积分、甲状腺左叶、右叶体积、血清TSH、TGAb、TPOAb水平以及峡部厚度降低,研究组更低(两组间治疗后相比)(P＜0.05).两组均无明显不良反应(治疗期间).结论 姜桂益瘿汤辅助西医治疗脾肾阳虚型桥本甲状腺炎有助于缓解患者的临床症状及体征,调节疾病相关血清指标水平表达,改善甲状腺体积和峡部厚度,提高临床疗效,且安全性高.
Abstract
Objective To analyze the effect of Jianggui Yiying Decoction(姜桂益瘿汤)on Hashimoto's thyroiditis with spleen-kidney Yang deficiency.Methods A hundred and forty-five patients with Hashimoto's thyroiditis admitted to Qinhuang-dao Hospital of Traditional Chinese Medicine were divided into the control group(72 cases)and the research group(73 cases).The grouping method was random number table method,and the selection time was from February 2022 to April 2023.The control group was treated with western medicine,and the research group was treated with Jianggui Yiying Decoction on the basis of the control group.The two groups were treated continuously for 12 weeks.The clinical efficacy after treatment and the occurrence of adverse reactions during treatment,the scores of TCM symptoms and signs,thyroid volume and isthmus thickness,serum thyroid stimulating hormone(TSH),thyroglobulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb)before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the research group was higher than that of the control group.After treatment,compared with those before treatment,the scores of neck infarction,neck swelling,anorexia,constipation,facial edema,mental fatigue,fear of cold,increased body mass index,serum levels of TSH,TGAb and TPOAb,volume of left and right thyroid lobes and isthmus thickness of the two groups decreased,and the results of the research group were lower than those of the control group(P＜0.05).During treatment,there were no obvious adverse reactions in both groups.Conclusion The treatment of Hashimoto's thyroiditis of spleen-kidney Yang deficiency with Jianggui Yiying Decoction is helpful to relieve the clinical symptoms and signs of patients,regulate the expressions of disease-related serum indicators,improve the thyroid vol-ume and isthmus thickness,and improve the clinical efficacy,with high safety.
