Incidence and influencing factors of exposure keratitis in ICU patients:A Meta-analysis
Objective To investigate the incidence and influencing factors of exposure keratitis in ICU patients,providing evi-dence-based reference for the prevention of exposure keratitis.Methods A computer-assisted search was conducted in databases in-cluding CNKI,Wanfang Data Knowledge Service Platform,VIP,CBM,PubMed,Embase,Cochrane Library,Web of Science,and CI-NAHL for studies on the incidence and influencing factors of ICU exposure keratitis published up to June 2024.Meta-analysis was per-formed using Stata18.0.Results A total of 14 articles involving 2 938 patients and 28 influencing factors were included.The incidence of ICU exposure keratitis was 30.1%(95%CI=20.9%~40.2%).Time of hospitalization≥5 days(OR=6.27,95%CI=0.97~40.58),treated with mechanical ventilation/tracheotomy intubation(OR=7.11,95%CI=3.20~15.8),Mechanical ventilation time≥7 days(OR=7.11,95%CI=3.20~15.8),time of mechanical ventilation(OR=1.13,95%CI=1.05~1.22),use of sedative(OR=2.74,95%CI=1.58~4.75),use of neuromuscular blockers(OR=0.41,95%CI=0.02~9.43),incomplete eye fissure closure(OR=10.89,95%CI=5.61~21.13),eyeball exposure(OR=5.73,95%CI=0.41~81.03),conjunctival oedema(OR=4.67,95%CI=2.20~9.91),perior-bital oedema(OR=5.73,95%CI=0.41~81.03),eyeblink reflex:5 times/min(OR=0.99,95%CI=0.01~82.13),other ocular symp-toms:ocular surface secretion(OR=11.04,95%CI=2.13~57.30)(P<0.05)were identified as influencing factors for exposure keratitis in the ICU.Conclusion The incidence of ICU exposure keratitis remains at a high level and ocular examinations on ICU day 5 and me-chanical ventilation day 7 can prevent the occurrence of ICU exposure keratitis.
