摘要
目的 系统评价音乐疗法对脑卒中后认知障碍(Post-stroke Cognitive Impairment,PSCI)的干预效果.方法 使用计算机检索中国知网、维普中文科技期刊数据库、万方数据知识服务平台、PubMed、Web of Science、Cochrane Library、Embase等7 个数据库中从建库至 2024 年 8 月 15 日公开发表的有关音乐疗法干预脑卒中后认知障碍的随机对照试验,由两名研究人员进行对文献的筛选、数据提取、质量评价,采用RevMan 5.3 软件进行Meta分析.结果 共纳入 10 项随机对照研究,涉及 798例病人.Meta分析结果显示,音乐干预组对PSCI患者的认知功能(SMD=1.26,95%CI:0.67,1.84,P<0.000 01)、日常生活能力(MD=7.88,95%CI:6.86,8.89,P<0.000 01)等方面均优于对照组.亚组分析结果显示,治疗疗程≥3 个月的干预,干预时长60 min的干预均较对照组患者的认知功能改善效果更好.结论 音乐疗法能改善PSCI患者认知功能、提高日常生活能力.
Abstract
Objective To systematically evaluate the effect of music therapy on cognitive impairment following stroke(post-stroke cognitive impairment,PSCI).Methods A computerized search was conducted for randomized controlled trials(RCTs)on the effect of music therapy on cognitive impairment following stroke,published from the inception of the databases to August 15,2024.The search covered seven databases:China National Knowledge Infrastructure(CNKI),VIP Chinese Science and Technology Journal Data-base,Wanfang Data Knowledge Service Platform,PubMed,Web of Science,Cochrane Library,and Embase.Two researchers inde-pendently screened the literature,extracted data,and assessed the quality of the studies.Meta-analysis was performed using RevMan 5.3 software.Results A total of 10 RCTs involving 798 patients were included.The meta-analysis results indicated that the music ther-apy group outperformed the control group in improving cognitive function(SMD=1.26,95%CI:0.67,1.84,P<0.0001)and daily living ability(MD=7.88,95%CI:6.86,8.89,P<0.0001).Subgroup analysis revealed that interventions with a treatment duration of≥3 months and those lasting 60 minutes were more effective than the control group in enhancing cognitive function.Conclusion Mu-sic therapy can enhance cognitive function and daily living abilities in patients with PSCI.
