Objective To analyze the causes of death with severe mental disorders from 2011-2021 and to determine the disease burden of patients with severe mental disorders.Methods Data were collected from"the mental health surveillance system of Gansu province"from 1 January 2011 to 31 December 2021 and analyzed.Results There were 19,553 registered patients with severe mental disorders and 2 539 deaths in Tianshui City,with a case fatality rate of 12.99%.The case fatality rate increased year-after-year from 2011 to 2016 and decreased year-after-year from 2016 to 2021(P<0.01).The case fatality rate peaked in 2016.The fatality rate for male patients was greater than the fatality rate for female patients(P<0.05).The mortality rates among patients were significantly different based on the following factors(P<0.01):age;level of education;marital status;occupation;economic status;and use of antipsychotic drugs.There was no sig-nificant difference in mortality based on nationality and family history(P>0.05).The main causes of death were physical diseases(51.36%),other causes(25.17%),mental illness-related complications(9.49%),accidents(9.18%),suicide(4.69%),and homicide(0.12%).The potential years of life lost caused by death of patients with severe mental disorders was 48,005.02 person-years,the average years of life lost was 18.91 years,and the potential years of life lost rate was 17.44‰.Schizophrenia caused the greatest life loss(34,972.02 person-years),and mental disorders caused by epilepsy had the highest average life loss(29.59 years).Conclusions The main cause of death in patients with severe mental disorders in Tianshui is shown to be physical disease,thus a comprehensive physical examination should be performed to ensure early diagnosis and treatment of physical diseases,strengthen the communication between the departments managing patients with severe mental disorders,accurately implement the rescue and treatment of patients,effectively reduce the mortality of patients,and improve healthy life years.
