摘要
目的 分析老年心血管病住院患者中衰弱的发生率及其相关影响因素.方法 选取2022年7-9月入住北京大学人民医院、北京中关村医院、北京第一中西医结合医院心内科的205例年龄≥60岁的老年心血管病患者作为研究对象.根据Fried衰弱量表评估患者的衰弱情况,同时收集患者临床病历资料和实验室检查指标,采用logistic回归分析老年心血管病患者发生衰弱的影响因素.结果 205例心血管病老年患者年龄60～95岁,平均年龄(72.48±7.87)岁,男100例(48.78%).衰弱患者95例,衰弱发生率为46.34%.Fried衰弱表型中前3位表型依次为握力降低、疲乏和步行速度减慢,分别为85例(92.39%)、84例(91.30%)和83例(90.22%).Logistic回归分析结果显示,年龄、体质量指数(body mass index,BMI)、白蛋白水平是衰弱的独立影响因素.结论 老年心血管病住院患者衰弱发生率较高,衰弱与高龄、低BMI和低蛋白水平相关.
Abstract
Objective To analyze the prevalence and factors influencing frailty in elderly inpatients with cardio-vascular disease.Methods A total of 205 elderly patients with cardiovascular disease≥60 years of age who were admitted to the Department of Cardiology of the Peking University People's Hospital,Beijing Zhongguancun Hospital,and Beijing First Hospital of Integrated Traditional Chinese and Western Medicine from July to September 2022 were selected as the research subjects.The Fried frailty scale was used to as-sess the frailty status of the patients.Clinical medical records and laboratory tests were collected.Logistic regression was used to analyze the factors influencing frailty in elderly patients with cardiovascular disease.Results The average age of the 205 patients was(72.48±7.87)years(age range,60-95 years)Of the 205 patients,100 were male(48.78%)and 105 were female(51.22%).There were 95 frail patients;the inci-dence of frailty was 46.34%.The top three Fried frailty phenotypes were reduced grip strength[n=85(92.39%)],fatigue[n=84(91.30%)],and slow walking speed(n=83[90.22%]).Logistic regression analysis showed that age,BMI,and albumin level were independent factors influencing frailty.Conclusions The incidence of frailty was shown to be high in elderly inpatients with cardiovascular disease.Frailty is associated with advanced age,a low BMI,and a low protein level.
