摘要
目的 系统分析维持性血液透析(maintenance hemodialysis,MHD)患者发生低血压的影响因素.方法 计算机检索中国知网、万方、维普、PubMed、Embase、Cochrane Library、Web of Science、中国生物医学文献数据库(CBM)数据库中关于MHD患者低血压的研究报道,检索时间为建库至2023年6月,采用Revman软件进行meta分析.结果 meta分析结果显示,年龄、心血管疾病、血红蛋白、白蛋白、超滤速度、血流量、透析年限、透析时间、超滤量/体质量、甲状旁腺激素、N末端B型钠尿肽前体(N-terminal B-type natriuretic peptide precursor,NT-proBNP)、血β2微球蛋白(β2 micro globulin,β2-MG)、糖尿病、糖尿病肾病、营养不良、超滤率、透析频率、低密度脂蛋白-胆固醇与维持性血液透析患者发生低血压均有关.结论 维持性血液透析患者发生低血压的影响因素众多,应尽早对维持性血液透析患者进行评估,对风险进行预防,避免低血压情况的发生.
Abstract
Objective To systematically analyze the influencing factors of hypotension in maintenance hemodi-alysis patients.Methods The databases of CNKI,Wanfang,VIP,PubMed,Web of Science,Cochrane Library,CINAHL and Embase were searched for studies on hypotension in maintenance hemodialysis pa-tients from the establishment of the database to June 2023.Revman software was used for meta-analysis.Results Meta-analysis results showed that,age,cardiovascular disease,hemoglobin,albumin,ultrafiltra-tion rate,blood flow,dialysis years,dialysis time,ultrafiltration volume/body weight,parathyroid hormone,blood NT-proBNP,blood β2MG,diabetes,diabetic nephropathy,malnutrition,ultrafiltration rate,dialysis frequency and low density lipoprotein-cholesterol were associated with hypotension in maintenance hemodialy-sis patients Statistical significance.Conclusions There are many factors influencing the occurrence of hypo-tension in maintenance hemodialysis patients.It is necessary to evaluate and prevent the occurrence of hypo-tension in MHD patients as early as possible.
