目的 探讨锦州地区体检人群的血清25-羟维生素D[25-hydroxyvitamin D,25(OH)D]水平,并分析其与体质量指数、血脂的相关性。方法 选取2022年1~12月在锦州医科大学附属第一医院健康管理中心进行体检的锦州市各企业员工及本地居民(市区及所属各市县)自发查体人员作为调查对象。测定调查对象的血清25(OH)D、体质量指数(body mass index,BMI)及血脂指标等检测结果,对维生素D充足组、维生素D不足组及缺乏组之间在年龄、BMI、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)的差异进行比较。应用Pearson相关、Spearman相关及多元线性回归对血清25(OH)D水平与BMI、血脂指标进行相关性分析。结果 本研究共有4 555人,其中男性占比55。7%,女性占比44。3%。25(OH)D平均水平为(21。93±7。47)ng/mL,其中男性为(23。78±7。29)ng/mL,女性为(19。61±7。04)ng/mL,两组差异有统计学意义(P<0。01)。研究对象中维生素D缺乏率为44。06%、不足率为42。22%、充足率为13。72%,差异有统计学意义(P<0。01)。三组之间的年龄及BMI、TC、TG、HDL-C水平相比,差异均有统计学意义(P<0。05)。而LDL-C在三组之间比较,差异无统计学意义(P>0。05)。相关性分析显示,25(OH)D与年龄(r=-0。237,P<0。01)、BMI(r=-0。064,P<0。01)、TG(r=-0。055,P<0。01)、TC(r=-0。118,P<0。01)呈负相关,与HDL-C呈正相关(r=0。085,P<0。01),与LDL-C无相关性(P>0。05)。进一步多元线性回归分析表明,女性、年龄、BMI、TC是影响血清 25(OH)D的独立危险因素(P<0。05)。结论 锦州地区健康体检人群血清25羟维生素D水平较低,因为维生素D水平较低是导致慢性疾病的一个重要风险因素,因此应及早发现维生素D缺乏,做好慢性疾病的一级预防工作。
Serum 25-hydroxyvitamin D levels in people with physical examinations in Jinzhou Correlation studies with body mass index and blood lipids
Objective To investigate the level of serum 25-hydroxyvitamin D(25(OH)D)in physical examination population in the Jinzhou area,and to analyze its correlation with body mass index and blood lipid.Methods Employees of various enterprises and localresidents(urban areas and their respective cities and counties)in Jinzhou who underwent physical examination in the Health Management Center of the First Affiliated Hospi-tal of Jinzhou Medical University from January to December 2022 were selected as the investigation objects.Serum 25(OH)D),body mass index(BMI)and blood lipid indexes of the subjects were measured.Age,body mass index,total cholesterol,BMI of vitamin D adequate group,vitamin D insufficient group and deficient group were determined.The triglyceride(TG),high density lipoprotein cholesterol(high den-sity lipoprotein cholesterol,HDL-C)and low density lipoprotein cholesterol(LDL-C)were compared.Pearson correlation,Spearman correlation and multiple linear regression were used to analyze the correlation between serum 25(OH)D level and BMI and blood lipid index.Results There were 4 555 subjects in this study,of which 55.7%were males and 44.3%were females.The average level of 25(OH)D was(21.93±7.47)ng/mL,and that of males was(23.78±7.29)ng/mL and that of females was(19.61±7.04)ng/mL,the difference between the two groups was statistically significant(P<0.01).The vitamin D deficiency rate was 44.06%,the deficiency rate was 42.22%,and the sufficiency rate was 13.72%,the difference was sta-tistically significant(P<0.01).There were significant differences in age,BMI,TC,TG and HDL-C among the three groups(P<0.05).There was no significant difference in LDL-C among the three groups(P>0.05).Correlation analysis showed that 25(OH)D was negatively correlated with age(r=-0.237,P<0.01),BMI(r=-0.064,P<0.01),TG(r=-0.055,P<0.01)and TC(r=-0.118,P<0.01).There was a positive correlation with HDL-C(r=0.085,P<0.01),but no correlation with LDL-C(P>0.05).Multi-ple linear regression analysis showed that female,age,BMI and TC were independent risk factors for se-rum 25(OH)D(P<0.05).Conclusion The serum level of 25(OH)D is low in healthy people in the Jinzhou area,because the low level of vitamin D is an important risk factor leading to chronic diseases,so vitamin D deficiency should be detected early,and the primary prevention of chronic diseases should be done well.
