探讨注意缺陷多动障碍(Attention Deficit and Hyperactive Disorder,ADHD)混合型(ADHD-C)及多动-冲动为主型(ADHD-HI)儿童在言语流畅性任务(Verbal Fluency Task,VFT)中脑血流动力学反应特征及差异.通过整合视听持续性操作测试将ADHD患儿区分亚型(33名ADHD-C型及36名ADHD-HI型),之后采用功能近红外光谱成像仪技术(functional Near-Infrared Spectroscopy,fNIRS)测量两组患儿在VFT中前额和左右颞叶血流动力学反应特征,并计算其积分值与重心值.结果发现,在注意力商数上,ADHD-C组得分均低于ADHD-HI组,差异有统计学意义(P<0.05);两组亚型患儿视觉得分低于听觉得分,差异均有统计学意义(P<0.05).在VFT测试中,ADHD-C组的左右颞叶积分值较ADHD-HI组降低,差异有统计学意义(P<0.05);两组亚型患儿左右颞叶的积分值与综合注意商、听觉注意商、视觉注意商均呈显著正相关(P<0.01).由此可见,ADHD-C与ADHD-HI亚型在完成VFT时的脑左右颞叶的功能存在差异,且注意力受损越重,其血流动力学激活程度越低.
Research on cerebral hemodynamic response under verbal fluency task in two subtypes of ADHD children
To investigate the characteristics and differences of cerebral hemodynamic responses in two subtypes of attention deficit and hyperactivity disorder(ADHD)children,combined subtype(ADHD-C)and hyperactive-impulsiveness subtype(ADHD-HI)under verbal fluency tasks(VFT)was used.Children with ADHD were divided into ADHD-C group(n=33)and ADHD-HI group(n=36)by integrated visual and audio continuous performance test(IVA-CPT).Characteristics of hemodynamic response in the prefrontal cortex and bilateral temporal lobes during VFT were measured by functional near-infrared spectroscopy(fNIRS).Their integral values and barycenter values were calculated.In terms of attention quotient,the attention quotient score of ADHD-C group was significantly lower than that of ADHD-HI group(P<0.05).Visual scores of all ADHD children were significantly lower than auditory scores(P<0.05).Integral values of the temporal lobes of ADHD-C type children were significantly lower than that of ADHD-HI type children under VFT test(P<0.05).When two subtypes of ADHD children were under VFT,integral value of temporal lobes was positively correlated with full-scale attention quotient,auditory attention quotient and visual attention quotient(P<0.01).The results demonstrated that the more impaired attention in both ADHD-C and ADHD-HI subtypes is accompanied with the lower hemodynamic activation in the temporal lobes during the VET.
