Actor-partner effects of subjective well-being and marital adjustment of Crohn's disease patients and Their Spouses

摘要
目的 利用主客体互倚模型探讨克罗恩病患者及其配偶主观幸福感对婚姻调适的作用.方法 采取方便抽样法,抽取南京市某 3 级甲等医院 210 对患者及配偶为研究对象,选用自制一般资料调查表、主观幸福感量表简表(Subjective Well-being Scale for Chinese Citizens,SWBS-CC20)、Locke-Wollance婚姻调适测定(Locke-Wollance Marital Adjustment Test,LWMAT)对其进行调查.采用独立样本t检验和卡方检验比较夫妻主观幸福感和婚姻调适方面的差异,通过主客体互倚模型和 Pearson分析法,分析夫妻双方主观幸福感对婚姻调适的影响.结果 患者和配偶各自的主观幸福感均可以正向预测自身婚姻调适(β=0.67、0.71,P<0.01),即主体效应明显;同时,患者与配偶的主观幸福感均能正向预测对方的婚姻调适(β=0.47、0.46,P<0.01),即客体效应明显.结论 克罗恩病患者及配偶的婚姻调适受自身及对方主观幸福感的交互影响,且主观幸福感与婚姻调适之间主体效应和客体效应显著.因此应将克罗恩病患者及配偶视为一个整体,关注夫妻双方的二元交互影响,积极探索以夫妻为中心的婚姻调适干预方案,对改善克罗恩病夫妻的心理适应有重要意义.
Abstract
Objective To explore the effect of subjective well-being(SWB)on marital adjustment in patients with Crohn's disease and their spouses using the actor-partner interdependence model.Methods A total of 210 pairs of patients and their spouses were recruited using convenient sampling from a tertiary grade-A hospital in Nanjing.They were surveyed using a self-designed general information questionnaire,Subjective Well-being Scale for Chinese Citizens(SWBS-CC20)and Locke-Wollance Marital Adjustment Test(LWMAT).The independent sample t-test or chi-square test was used to compare SWB and marital adjustment differences.The actor-partner interdependence model and Pearson analysis were used to analyze the influence of SWB on marital adjustment.Results The SWB of both patients and their spouses could positively predict their marital adjustment(β=0.67,0.71,P<0.01),indicating obvious actor effect.The SWB of both patients and their spouses could positively predict the marital adjustment of each other(β=0.47、0.46,P<0.01),which showed obvious partner effect.Conclusion The marital adjustment of Crohn's disease patients and their spouses is affected by the interaction of their own and their partner's SWB,and the subject effect and object effect between SWB and marital adjustment are significant.Considering the patients with Crohn's disease and their spouse as a whole,developing a marital adjustment intervention program and focusing on the binary interaction of the couple are important to improve the psychological adjustment of patients with Crohn's disease and their spouses.
