右美托咪定联合地佐辛对糖尿病足手术患者坐骨神经股神经阻滞效果的影响

摘要
目的 探究右美托咪定联合地佐辛对糖尿病足手术患者坐骨神经股神经阻滞效果及血糖的影响.方法 选取2020年7月至2022年8月于山西医科大学第二医院收治的120例糖尿病足手术患者为研究对象,根据随机数字表法随机分为对照组(60例)和观察组(60例),其中对照组给予地佐辛治疗,观察组给予右美托咪定联合地佐辛治疗.比较两组坐骨神经股神经阻滞效果及血糖水平.结果 T1、T2、T3时VAS 评分观察组分别为(6.87±1.33)分、(3.41±0.59)分、(4.23±1.22)分,对照组分别为(7.48±1.52)分、(5.30±0.75)分、(5.87±1.30)分,两组相较于T0时均显著降低,且观察组相较于对照组显著降低(P<0.05).观察组运动神经、感觉神经起效时间为(22.31±1.52)、(18.20±1.20)min,对照组为(23.91±1.60)、(19.32±1.35)min,观察组较对照组显著缩短(P<0.05).观察组运动神经、感觉神经维持时间分别为(382.91±60.16)min、(432.18±65.33)min,相较于对照组的(332.10±52.18)min、(390.11±59.27)min显著延长(P<0.05).两组T1、T2及T3时间段收缩压、舒张压水平及心率相较于T0均显著降低,且观察组相较对照组显著降低(P<0.05).观察组不良反应发生率相较于对照组无明显差异.结论 右美托咪定联合地佐辛作用于糖尿病足手术患者可有效镇痛,延长神经功能阻滞,维持血流动力学的稳定.
Abstract
Objective To explore the impacts of dexmedetomidine combined with dizosin on sciatic nerve-femoral nerve block and blood glucose in diabetes foot patients undergoing surgery.Methods A total of 120 dia-betes foot patients underwent surgery who were admitted to Second Hospital of Shanxi Medical University from Jul.2020 to Aug.2022 were selected as the research objects.The anesthesia method was sciatic nerve block-femoral nerve block,and were randomly grouped into the control group(60 cases)and the observation group(60 cases).The control group was treated with diazosin,while the observation group was treated with dexmedetomidine com-bined with diazosin.The effects of sciatic nerve block and femoral nerve block,and blood glucose level were com-pared between the two groups.Results The VAS scores at T1,T2 and T3 in the two groups were obviously lower than those at T0,and the VAS scores in the observation group were obviously lower than those in the control group(P<0.05).The onset time of motor nerve block and sensory nerve block in the observation group was obviously low-er than that in the control group(P<0.05).The maintenance time of motor nerve and sensory nerve in the observa-tion group was obviously higher than that in the control group(P<0.05).Compared with T0,systolic blood pres-sure,diastolic blood pressure and heart rate in T1,T2 and T3 time periods in the two groups were obviously lower,and the observation group were obviously lower than the control group(P<0.05);The observation group had no sig-nificant difference compared with the control group in terms of the incidence of adverse reactions.Conclusion Dexmedetomidine combined with dizosin can effectively relieve pain,improve nerve function block,and maintain the stability of hemodynamics in diabetes foot patients undergoing surgery.
